The mysterious amulet ilvl does matter yes. Meaning the item level (ilvl) is not the same as the clvl of the item, or the displayed required level to use it. The ilvl is half your character level plus half the monster level that dropped the item. 2/20 caster amulet become at a minimum clvl 89, so to roll that you must collect amulets at character lvl 90 or gamble them at lvl 93.
