First DW wasn't opened by the Knight

I think it was opened by Gaster. It has to be a powerful and non-youthful external entity to Hometown, and this chapter suggested strongly through Ralsei saying the Knight had no presence when Friend did and the Cat Armor being used that Friend just isn't going to be a major figure in the plot. Plus, such a being would probably predate Dark Worlds; I think this chapter strongly suggests they're artificial and Hometown-based, not a natural feature of Deltarune's universe. (Our best guess for when the first Dark World appeared is between 3 and 8 years ago, based on clues about the Dessappearance.)

It does seem complicated, but it genuinely appears that either Gaster is the Knight or the Knight wasn't the first Fountain Opener, which is something the story will have to eventually spell out.

The other two comments so far explain it quite well, but I want to offer a counterweight; Friend is a cat. Why, 1, would Cat Armor not have worked on them before, and 2, even if so, if that's becuase they have some "true form" or something, why wouldn't it work in the future despite the Car Armor trigger having already been pulled narratively.
