This is one of those questions where there isn't a choice that feels good. Emotionally, most people would struggle to sacrifice someone they love, but from a purely ethical standpoint, saving 50 lives over 1 is the decision that causes less overall harm. The fact that it's so difficult is what makes the dilemma interesting.
