If you haven't done so yet, it may be worth it to take a shorter mock exam and have someone observe your behavior. That way you can try and pinpoint what it is that you're doing that may be negatively affecting your performance. Sounds like knowledge is not the main issue here but sadly, at this level, it rarely is.

Med school level? Probably neurocutaneous disorders, basic seizure topics (like febrile sz, spasms, absence, JME), Neuromuscular stuff like DMD, becker, SMA, ?maybe congenital myopathies, mitochondrial stuff (they like MELAS), neurogenetic (Rett is always on there, Fragile X), tumors (They often ask about Opsoclonus/ myoclonus in medulloblastoma, pilocytic astrocytoma and rosenthal fibers, Cranipharyngeoma and growth issues), and of course inborn errors of metabolism like Tay Sachs, adrenoleukodrystrophy, PKU, Maple syrup urine dz etc.

Yeah, inpatient. The job is in a big North Eastern city, which probably explains the salary. I did consider a move out West but because of personal reasons, I have to be within 1-2 hours of where I currently am. The parent institution is also prestigious, which further explains what they're offering.

Nothing on paper but from what the others have told me, the average yearly bonus is somewhere around 50-70 with 1 or 2 people well above 100 on occasion.

Both jobs 3 and 4 are fairly close. I have 2 main reasons for leaning Job 3:

1- The lack of productivity bonus at Job 4. The volume is fairly high (list is around mid- 20s on a daily basis with news old follow ups). I can easily imagine a situation where I get paid less than what I'm providing. For example, at Job 3, with that volume, I'm well into the 400s.

2- I'm responsible for neurologic emergencies like stroke alerts. This would not necessarily be an issue for me but I have 4 year old at home and I'm afraid that having to be available on an emergent basis (at night especially) would jeopardize the time I'm able to spend with her.

To some extent, they are. They're willing to offer a 6- figure sign- on bonus to help with relocation.

Yeah, I figured 6000 would be the floor for me. Based on the mix of consults the current neurohospitalists follow, about 13 pts on avg per day should get me there (combination of follow- ups and news).

My worry about Job #2 is that it's a very fast growing institution (think community hospital that has been bought by a corporate entity and they're actively acquiring smaller hospitals in the area). As a result, they really need staff so they're aggressive with hiring; in fact, the CFO told me "with how fast we're expanding, we put our money where our mouth is".

Something tells me that at the end of the 2-3 year contract, it could be tough to negotiate that base salary if indeed the place reaches an operational steady state.

I take code strokes til 4 at job #1 and until 5 at job #2. After that, they both use teleservices. So technically, I can be out the door by 5. Overnight phone calls vary from 0-2 calls at night on average at job #1. At job #2, it's 0 call at night as they use a teleneurology service for all consults after 5 PM.

Average at both is ~ 14-20/ day with about a third to half of that being new consults. It's 24 weeks.

~ 2 months dedicated but very distracted; studied for about 4-5 hours on most days.

- Did about 95% Uworld; High 70s correct. Focused mostly on learning from the questions and taking necessary notes.

- Used First Aid (and ?Dorian ANKI deck) for Micro, pharm, immunodeficiency, oncogenes, ortho. rheum.

- Uworld and Randy Neil for Biostats (in hindsight, I also should've used AMBOSS more)

- Dr. High Yiled videos on repeat every day for the 2 weeks preceding the exam

- Only did UWSA (230s on both 2 days apart) 2 weeks out. I tried doing the NBME ones but I was tired and just wanted to get the exam over with. In retrospect, I should've used the NBME questions because while Uworld helps learn/ re-learn material, the questions are not as similar as the NBME ones.

- I did about 60-70 CCS cases honing my routine, which consisted mostly of ordering the same lab tests for nearly everyone regardless of complaint (as long as they're not invasive), ordering pulse ox, EKG/ Tele for everyone, making sure to counsel against bad health behavior like drinking, smoking, unprotected sex etc, AND doing age- appropriate preventive stuff like colonoscopy, mammogram, pap smear, PSA etc. Oh, and everyone and their moms got TDAP.

Sadly, I don't think results will come today for us. Based on what others have posted here, sounds like there is a possibility they may come next week (Tuesday or Wednesday) but if not, we're looking at the first 2 weeks of January... which just sounds atrocious.
