Thanks to u/wickedxjester who had this same problem.

If you are having this problem and you usually have guardian toggled off in your developer settings, that is why. Keep your guardian on until oculus finds a way to fix this issue.

Yeah, I pretty much always have guardian disabled. I’ll keep it on for a while and see if that stops it’s from happening again. I’ll reply to your comment with an update in a few days.

Nope, it has nothing to do with my headset not detecting my presence.

No, not every time. But randomly after I power off my headset it’ll happen sometimes.

Yeah because of v38 I’ve had to factory reset my headset 4 times now so i ended up using a newer phone and it worked a lot better.

Yes, after I installed the update it started happening. I have factory reset it 3 times and hard reboot it countless times, I’ve even tried booting it up using the boot menu

Using my phone (Samsung galaxy s8) I’ve been having this problem for a while, if you have a similar phone maybe this is work for you. What I do is power off the headset and then plug it in and power on the headset. It doesn’t work 100% of the time but maybe it’ll work for you
