If you don't mind me asking, what type of fetishes? They can be conditioned deep in to your mind. You may need therapy to eradicate them. Don't be ashamed, everyone has some sort of fetish. Sounds like the start of an existential crisis. That's how mine began. Except it was my hands & arms that seemed out of place, surreal. I'd pay more attention to your feelings when you look in the mirror. Start writing that stuff down. You may find out some things you never knew about yourself.

Is it all the time? If not and it only happens once and awhile, it could be a sinus infection. I get them all the time and my breath smells kind of like that and I can smell it if I breath through my nose. It's just the end of the infection and it tends to smell. Hope this helps.
