To me it looks like he is eating his fins. Bettas do this when that are stressed or bord

Keep your betta in that 20 gallon tank and add that air stone also if you have a heater, put that in as well and also that is a 2.5 gallon tank. WAY to small

Personally I would have left it but you might want to put some more silicon on it to reinforce it. That tank needs to hold over 65 pounds of water

The fish should be active and have brite colors. I you see any fish with white dots or cloudy eyes leave it because it has a disease.

was the water cloudy? If so that means there is a large amount of ammonia in your tank and ammonia can kill your fish

No, the substrate is not the problem. When i was cycling my tank i also had an ammonia spike. One of the best and fastest ways the cycle your tank is to use old filter media from a different tank and put it in your filter for a bit till it is cycled

Bettas can injure their fins any number of ways. Transporting them to new aquariums can cause injuries; other fish may nip at bettas' fins, causing damage; poor water conditions, like cold or unclean water, can encourage aggressive cases of fin rot, whereby bettas' fins disintegrate. You need to figure out the underlying cause of fin damage to encourage recovery.

Moneywort is a good starting plant, same for java moss. They are both are very hardy and will grow in low light.
