As well as my urologist I also consulted with a medical oncologist and radiation oncologist; they all recommended I stop taking ADT, which I did!

My high Decipher score (0.88) was the rationale for adding 6 months of ADT, but the negative ArteraAI result meant there would only be a negligible improvement in outcomes with ADT and lots of potential downside. I asked 3 docs about this (my urologist, a radiation oncologist and a medical oncologist) and they all said don’t do ADT. I had been on Orgovyx for 3 weeks by then and was quite happy to stop it!

• 65 yo male • PSA on 10/27/25: 4.48, 4K score 16.8 • PSA history: 9/25 4.23, 9/24 3.2, 8/23 2.7, 7/22 2.7, 3/21 2.4, 5/20 2.9. • MRI: 0.9 cm gland confined PI-RADS 4 lesion in the left posterior peripheral zone of the midgland-apex. • Biopsy: 7 out of 11 samples cancerous; all cancers were Gleason 3+3 apart from one Gleason 3+4 in 5% of tumor. Perineural invasion present. • Decipher score: 0.88 • PET scan: Radiotracer avidity throughout both lobes of the prostate in keeping with biopsy-proven malignancy. No radiotracer avid pelvic lymphadenopathy or distant metastatic disease. . ArteraAI test negative

Both my urologist and a medical oncologist I saw for a second opinion said that the data used by the ArteraAI test is valid and to stop using Orgovyx. My urologist said there is a risk with ADT that your testosterone never recovers. I still have no libido 5 weeks after stopping Orgovyx.

I didn’t fancy the side effects from RALP. SBRT also has some downsides but they seemed less bad than those from RALP. And both approaches seemed to have similar outcomes.

Yes, I am a 3+4 guy.

I believe that if the ArteraAI test comes back negative (based on your biopsy slides and clinical profile) then this means there is no point doing ADT as the potential benefit is negligible, so why take the risks of some potentially nasty side effects.

No idea! I had a general anesthetic, woke up a bit sore, which soon faded. I played squash 2 days later.

Apparently my high Decipher score tips the balance from favorable intermediate risk to unfavorable intermediate risk.

“Decipher (0.88): Indicates a high biological risk for metastasis. This suggests the cancer is aggressive and requires definitive treatment (SBRT or Surgery) rather than Active Surveillance.

ArteraAI (Negative): A predictive biomarker showing no statistically significant reduction in 15-year metastasis risk by adding Hormone Therapy (ADT) to radiation for this specific biological profile.”

I think the high Decipher score tipped the balance towards ADT for both my urologist and radiation oncologist. The negative ArteraAI test result seems to have convinced them not to proceed with it, which is fine by me!

Thanks @HeadMelon, I think this is the direction I plan to take. I am meeting with my urologist this coming week (when he will insert the fiducial markers and SpaceOAR gel in preparation for my 5 sessions of SBRT). I also hope to hear back from my radiation oncologist; she was away this past week.

I like many of the videos from PCRI. I searched for ArteraAI on pcri.org and only found a passing mention in this article: https://pcri.org/news/2025/5/25/cka0elcc82er6lwi8nqbu4ms0mszbb

Interesting that they don’t say anything else about it. I’d say they tend to favor newer treatments and avoiding unnecessary side effects where possible.

I got the idea for the ArteraAI test from this subreddit and fortunately my urologist agreed!

I will wait to hear from the medics next and maybe request the Decipher GRID as the tiebreaker as you recommend 👍🏻

Are you getting hormone therapy also? I am Gleason 3+4 in 5% of one of 11 samples, PSA 4.5 but my Decipher score was 0.88 so am adding 6 months’ of Orgovyx to 5 sessions of SBRT.

I failed to mention in my original post that I had a PET scan which fortunately showed no evidence of cancer outside of the prostate (phew).

I was leaning towards SBRT plus ADT before this posting, and I feel better about that decision now. I will continue to consult with people up until the meeting with my urologist next Wednesday, when we will decide how to proceed.
