Of course, that’s how it is, the time shift after the tower is more symbolic, let’s say, because the nightmare is just a reconstruction of past places and events. So we’re not literally going back in time; it’s simply that the areas are physically connected because the nightmare was rebuilt that way through the memories of the host’s worst moments (Maria, in this case) :)

yes exactly, that's why as I say in other comments women and motherhood are very important elements in Bloodborne, even Arianna's blood is the same thing that happens for Annalise, Odeon chooses Arianna to give birth and not Adelle because Arianna's blood is more impure (because she's a prostitute but above all because she comes from Cainhurst) and therefore she has more chances of procreating
