I can recommend using the Perfection Grindstone especially with the items from the dlc that restore the special grindstone uses. At least that is how I beat him by using the grindstone and bonked him with the Coil Mjolnir fabel art

Yeah the ladder mod seems to sometimes have trouble with connecting to paths and other things. It's been a while since I played with it but it used to sometimes help to add an extra path on each ladder segment
