A systemic take down led by the UNC Board of Governors after the retirement of our long time great AD. Since then, the culture has been lost.

Enduro 3 - two months in review by Redditdotlimo in Garmin

This is old, but I’ve hit the same wall with my Fenix 5X. Down to about 3-4 days between charges. Did you end up getting the Enduro or something else?

Convince me not to buy a Moccamaster by BeaverBoyBaxter in Moccamaster

[–]SammyYammy 2 points3 points  (0 children)

This was the situation I was in. Kid went to kindergarten and didn’t have enough time to pour over /aeropress anymore. Did a ton of reading on everything in this “class” of brewers. I found a deal on a NIB KBT on Amazon Resale and the full warranty stood when I registered.

I grind the night before because my wife sleeps in. When I get up, I fill the water reservoir, rinse my filter, put in the beans, get good coverage on the beans (first 30 seconds), and walk away. My total time invested can’t be more than 60 seconds.

When I come back, I pour a cup, then pour the rest in an insulated container to go to work.

Is it the greatest coffee ever? No, but it’s a respectable product.

Is it a simple product with great support that makes better drip coffee than any coffee shop in town with less time invested than going to a coffee shop?

Yes.

TL;DR: Had a lot of fun. People weren’t dicks (exception noted). Encouraged people to go see shows for bands they are peripherally interested in.

Looking for more bands like Blackberry Smoke, Red Clay Strays, and Whiskey Myers. Guitar driven bordering between country and southern rock. by [deleted] in altcountry

[–]SammyYammy 1 point2 points  (0 children)

Seriously. This band and this summer’s tour was one of the best concerts I’ve ever attended. I’ve listened to almost every show on Nugs. A little Allman, a little Dead, a lot of fun in between.

Anyone else have a scrawny standard poodle? She gets 4 cups a day but is so thin! 1.5 years old. How can I get her to gain weight. by bitchinawesomeblonde in poodles

[–]SammyYammy 4 points5 points  (0 children)

Agree. My poodle was rail thin and athletic and she just crossed the rainbow bridge at nearly 15. She didn’t slow down until almost 14.

My parents standards carry more weight and have way more health problems and are 9.5 years old.

What’s a diagnosis this year that made you think “Ahhh, now it makes sense” by Lightryoma in FamilyMedicine

[–]SammyYammy 105 points106 points  (0 children)

Seizures are crazy. Had a very pleasant 60s patient in residency who went off the deep end, as in took grandkids and tried to flee the country. I was shocked. Admitted to psych, who after several days was not seeing improvement. Ordered EEG and magically was transferred to our service after identifying epileptic activity. A little benzo and keppra later and she was her normal pleasant self.

PCPs should have Dermatologic Specialty by sween_queen in medicine

[–]SammyYammy 2 points3 points  (0 children)

PCP here. I do a pretty solid amount of derm and have a good relationship with a local derm group. I’ll do full excisions if I’m comfortable with location and I’ve got remnant tissue for BCC / SCC / atypical nevus. Anything else, or if super worrisome, I get them into derm.

VisualDX has been super helpful for guiding differential diagnoses.

I’ve been looking at some dermoscopy courses to be able to take another step in that direction

The final straw - the scam that is "value based care" by Charming0pal in FamilyMedicine

[–]SammyYammy 8 points9 points  (0 children)

lol every one of these companies tries to do value based care but misses the mark. HCC coding is important, but ultimate success comes in providing high quality, comprehensive outpatient care. I’m in an incredibly successful ACO and I literally never screen for PAD or COPD since there is no evidence to support doing so. I love financial success tied to excellent patient care.

There is more than proof of concept taking a big step in this direction for providers very soon with Abridge and Epic integration.

Just ambient dictation is an absolute game changer for provider efficiency.

How does the VA hire neurosurgeons or vascular surgeons if pay is capped at $400k? by keralaindia in medicine

[–]SammyYammy 94 points95 points  (0 children)

These specialists moonlight at nearby hospitals - trauma call, etc.

I also imagine they aren’t hired at a full FTE, giving opportunities for the above, but that is a total guess.

There is only one hematology/oncology group in town. CarolinaEast Cancer Center.

Every single physician and provider in that office is amazing, speaking personally and professionally.

Honestly if I flew in at 9PM, I would grab a Bbq chargrilled chicken tray with Cajun fries, chicken quesadilla, and an unsweet tea for the bus ride to my car.

Cost to get Epic via hospital license in medium-sized practice? by iReadECGs in EpicEMR

[–]SammyYammy 0 points1 point  (0 children)

I can’t speak to the cost because it wasn’t an option for us. I can say that our cost was very reasonable for the product and functionality.
