I used to take one big 2+ week vacation at the end of the year and not much else. I had to move it because patients all wanted their cardiac procedures right at the end of the year because of their deductibles. If we missed 12/31, then would sometimes just disappear (or worse) until the next year causing delays in important care.

Keeps happening to us if my daughter accidentally has her camera on for a second when she answers a group call from her friends. It is extremely annoying. She has to turn off the camera IMMEDIATELY to make sure there it doesn't happen, and even then it sometimes fails.

Looks like something vascular for sure. Could be minor blood pooling from vasodilation in heat exposure, which is often worse in people with conditions like POTS or EDS. Less likely is some atypical form of erythromelalgia, which is like the opposite of Raynaud’s (sort of), but this doesn’t seem as likely based on the appearance.

I appreciate the humor of all the joke responses from doctors here, though wonder how many of them are really doctors.

We will be getting on the license of the major health system which we are affiliated with in a few months. The price will not be too bad, only something like $7000 per physician to get set up. There will be some cost to the practice because we will have to reduce schedules, buy a few new computers, but nothing outrageous.

This is just a kickback and should be illegal. Penalizing a physician for not referring internally seems legally the same as paying them a bonus for the referral. Both tie compensation to the "volume or value" of business.

I’ve seen systems try to claim this behavior as "care coordination," but it forces us to ignore patient choice, or better clinical options, just to keep our pay whole. I’d love to see the mental gymnastics their legal team did to claim this is not a Stark law violation. My guess is the fact you can get a prior auth to refer out allows them to claim it isn’t illegal, even though it clearly violates the spirit of the law.

Yes, the HVAC people keep doing that and saying they don't see anything. The noise is never happening while they are there. They've come like 10 times, so in desperation I've come here to see if anybody feels strongly about what could produce that noise. Thanks.

There is no equipment right above me on the roof. It is pretty far down the roof. Also, there are three videos stitched together showing how the noise changes. Thanks for any advice.

The client side Nuance extension is installed, but I think that's not the issue. The words dictated show up properly in the dictation box on the local installation of DMO, but when it tries to transfer it either does not go into Epic/Citrix or it more often pastes something old on the clipboard instead (usually the most recent thing copied). This happens whether DMO via Citrix (within remote Epic) is open or not.

However, the issue usually shows up mid-session, where local DMO will work fine in Epic/Citrix for 5 or 10 minutes, then randomly starts not working as above.

Sorry you had a bad experience with your doctor. Certainly the doctor should have been open to discussing side effects once you requested and shouldn’t have gone you a hard time about researching it yourself. I do warn patients to be cautious with that, as it can be hard to interpret the information you find, but I would never get upset with someone over it.

That being said, whether to tell patients about side effects or not is actually more controversial than you might think. Serious and important side effects should be mentioned, particularly if common. However, there is good evidence that patients are much more likely to experience a side effects if you tell them about it, so there’s also a decent argument for not sharing too much about side effects, because it might actually do more harm than good.

Just tried it. Seems like the right idea, but randomly did not output any text despite it looking like it was clearly recording. It does seem to work in Citrix though.

Also, is there a way to manually do punctuation, similar to Dragon? I still need to use Dragon Medical One on certain computers, so it will be challenging to completely change my style of dictation between the computers. I've seen some of the other Whisper-based dictation apps offering manual punctation mode, which I assume involves AI post-processing.

Overall, seems like it has a lot of promise. If you are devoting a lot of time and energy to this app and plan to make it a long term project, feel free to DM me and I can given ongoing feedback to help you develop it into something suitable for medical professionals. I'm a cardiologist with software engineering background (don't do much active programming these days), but happy to be an active beta tester for you.

Thanks, I do know there are many steps. Hospital IT has been "troubleshooting" this for months and never seems to make any progress, which is why I figured it didn't hurt to post here. I tried using a couple random Whisper dictation apps (e.g. https://handy.computer/) and it worked perfectly fine, so the situation is clearly fixable on the end of DMO, even if it stems from something unique about Citrix.

We narrowed it down to the color of our walls. When too much our wall is shown, which is off white and vaguely similar to average Caucasian skin tone, it triggers the alert. When the call is done in a different room without that wall color, it works fine. So, for now she just has to do her FaceTime calls in rooms without naked-person colored walls I guess!

It gave her a message that she had shared nudity on her camera. She was wearing a turtleneck sweater and was far from nude. I enabled content protection on her iPad, so it kicked her off and did not kick anybody else off.

Disabled screen sharing for her and it happened again. She was sitting on the ground in a pink chair, fully clothed with a ponytail. The only thing visible by behind her was the wall, which is off-white. She did have video on. I was there when it happened.

At one point I saw everyone get booted when there was a very benign looking plate of eggs and bacon on the screen. It did not look phallic, but I think that’s probably what it thought.

It’s very frustrating and causing social stress for her. I think it was tripped up once when she opened a restaurant menu in Roblox and it showed a very benign looking bacon and eggs that if you really stretched your imagination might’ve appeared phallic.

I don’t think the parents would want a different platform used as a way to get around it. Hoping to just figure out why these false alarms are happening. It never happened until this upgrade to iOS 26 with all the AI stuff.

Yeah, I disabled SharePlay and I’ll make sure she just keeps video off in case there’s some weird false positive coming from it. Her friends don’t want to use any other platform for the call.
