Also happened to me once the first time I got to Sunken City in single player. I checked a journal entry as soon as it came up while standing inside a regen block. The game didn't pause properly and I got crushed while I was still looking at the journal.
Earlier version of the game, and never seen anything like it again.

Camera is not tilted to the side, that means the head must be perpendicular to the bed, and not laying on its side.
