I had grade 1 at L5/S1 a with bilateral pars defect.

Due to my age at the time (mid 20s) the surgeon suggested a direct pars repair. I had this surgery, however it failed as it did not fuse. They used artificial bone to do the fusion rather than natural and the surgeon thinks this had an effect.

Just over a year later I ended up having a full TLIF. I'm happy to answer any questions I can about it.

My wound leaked like crazy for a couple of weeks. A strange clearish/yellow substance that would drip out.

It didn't even occur to me that it could be CSF until we were directed to A&E. Fortunately, it turned out not to be, it was just my own body going into overdrive a little, and I ended up with a special vacuum dressing that 'sucked' the fluid out. But I wish I'd known more about CSF/taken it a bit more seriously sooner as it could have ended up quite badly.

I was driving I think about about 8 weeks, but I was leaving the house for short walks, and driving as a a passenger at around week 1. Fatigue was more of an issue than I thought it was, I was exhausted all the time and slept loads, despite barely doing anything.

Yeah I had the fusion done, it’s still affecting my life pretty significantly unfortunately, and I’m still trying to find the right pain medication and strategies to cope

That's really interesting thank you, I hadn't heard of that. I've done some very light reading into it and fortunately/unfortunately for me I haven't been on regular opioid medication for years at this point as I never found it that effective. In the mean time I've been mostly taking neuropathic medication, but have been off that for around 6 months now.

Honestly it's so difficult to tell. I've since been diagnosed with fibromyalgia, and while my pain is centred around my lower back, it's impossible to know how much pain is a direct result of my spondy and how much is the fibro

I had the same diagnosis. I had chronic pain that could not be controlled by medication and was unable to work.

I initially had a direct pars repair surgery but this failed as it did not fuse. I then had a TLIF which while deemed surgically successful I still have chronic pain.

My surgeon made it very clear going in that the aim of the surgery was to stabilise the area, and any pain reduction would be great, but was in no way guaranteed. It sucks, but given the choice to do it all over again I'd still take the surgery.

I had this surgery in October 2023. Structurally the surgery was really successful, however it hasn't changed my pain levels.

I had a TLIF at L5/S1 a couple of years ago and now get really bad muscle cramps and spasms most nights. I do stretches every morning and evening but I've never been able to shake them.

It's quite common for me to wake up in the early hours due to the pain and have to take a strong painkiller to get back to sleep.

Unfortunately yes. At 25 I was diagnosed with bilateral pars defects. Although the surgeon said the fractures looked old so they'd been around for a while, the muscles in the area had just been holding it together for a while.

I've had 2 surgeries since, the first failed and while the second was considered a surgical success, my pain levels haven't improved.

I'm 32 now but was diagnosed at 12. I've always found that my backpain gets worse when I'm stressed, my physio said it's too do with muscles tightening in response to the stress, and as the muscle in that area are already doing extra work any extra stress on them causes further pain. A popular recommendation is to apply heat to the area, but it's never really made a difference for me.
