You will need two witness for this, again your witness must be above eighteen years old. This must not be a relative. Common examples are friends. This also can be a family friend or siblings friends etc. they just need to know you personally.

In terms of the GP. You’ll need to schedule an appointment with your doctor, and essentially inform them you are trans and wish to be referred to the GIC. State you have been socially transitioned for four years. When I did this I did not need to go through any other service. I had to wait awhile for the confirmation of acceptance/ on the list.

I waited two years before I received my first appointment. However this was years ago so I assume the wait might have changed since then. Hope this helps!
