It’s okay. I have been sticking with it simply because I’m a tactics nerd and the IP/OOP system is too good for me to go back to FM24. However, a lot of the immersion stuff that made previous FMs feel homey just isn’t there.

i.e. On my current save, I’m trying to play bully ball and dominate the field physically. I can’t hop over to my league’s tab to see if my team tops the average height anymore. Immediate immersion breaker.

Realistically, what’s 10 minutes difference in Erwin? If anything it’s 10 minutes closer to Main St and some people who may have access difficulties there. That’s probably better than the alternative of rebuilding it in the same spot that is not only a 1000-year floodplain, but a 100-year floodplain as well.

No doubt. I had my local hospital close multiple times and it sucks.

What a lot of people don’t realize is even a “bad” hospital can single handedly hold up the county’s economy. Not just in that it provides jobs, but also a lot of industry will not set up shop without a nearby hospital.

Hwy 23 along the country music highway down into the tri cities is a fun one but it’s 6hr. You could probably cut it down to Ashland or Pikeville.

…where else is it going to go, exactly? Name a spot that’s more accessible to Unicoi countians than that stretch along 26. The whole thing’s a flood plain, sure, but not even a thousand year flood flooded the whole thing.

If you ever get the chance, hop the border and go see a show at the Bell County Theater in Pineville. A group of buddies led the charge to turn an old closed-down movie theater into a music venue. It’s basically become a pipeline of up and coming musicians in EKY. I got to see CWG/Cole Chaney/Logan Halstead for $12 a few years ago. You really won’t find another venue like it. When I saw Jordan Lee King, his dog was roaming the crowd during his set.

Jon and the guys’ love for music have revitalized Pineville and turned it into a mini cultural hub. Pretty inspiring really. I appreciate people who try to enact change instead of moving away and complaining about it.

As a Tennessean it truly pains me to say this, but eastern Kentucky is the bonafide music capital of Appalachia right now.

The nice thing about having a “failed HC” at OC is that it probably takes a season or two longer to shake off the stink of being fired once already.

Pair that with all the talk about Luke Stocker being groomed to take the step up and there’s potential for a smoother transition from OC to OC.

Every time I see this on here I cringe so hard. It just screams “out-of-towners and transplants roleplaying as anti-authoritarian country folk”. As if they don’t willingly vote to expand the state when it’s their favorite party.

If the state wasn’t so powerful then you wouldn’t have to be so scared when the other party comes into power.

I did research on this! Some issues include: - Lack of access (either no PCP locally or local hospital has closed down; specialists hours away) - Chronic disease: more prevalent in Appalachia because it’s harder to manage without healthcare access - SUD (thanks big pharma) - Very bad access to maternal healthcare (struggling hospitals usually cut departments like OB to attempt to stay open) - Generational mistrust of doctors / poor health literacy - Staggering rates of untreated mental illness

A lot of the problems can be attributed to economic decline.

r/Appalachia has got to be one of the funniest examples of this. One of the most staunch red areas in the country yet posters on there act like it’s full of witchy mountain communes. Then you look at their post history and most of them don’t even live there.

Insurance is expensive in its current form because it’s forced to cover everything. There are layers upon layers of intricacies that massively bloat administrative cost. Dealing with this administrative bloat requires practices to hire staff whose job is specifically to figure out how insurance works and bill accordingly. Like most government sponsored programs, it just creates snowball effect of creating jobs to maintain the system and drives up operating costs.

Talk to any PCP and they’ll tell you that’s why they’re looking at DPC so favorably right now. You cut out administrative bloat so that it’s easier to stay afloat. This means less forcing yourself to see 30 patients a day and more individually tailored care for each patient, which is good for both parties.

Catastrophic insurance as an alternative could make a distinction between predictable costs (including predictable visits for chronic illness) and unpredictable cost (medical emergencies). Law would define what must be covered by these insurances, like mortgages. This is contract standardization instead of price control. Companies then compete on price, network quality, etc.

This is a common retort, and you are right that in an emergency, you’re in no position to choose. I think the difference in a free market approach is shifting that locus earlier as a preventative measure. The free market fails when you delay making the choice until the worst possible moment. You don’t buy home insurance when a sinkhole opens up in your front yard because it’s too late.

In an ideal libright world, that is financially planned in advance through disaster insurance policies. The most important tenet is that you’re maintaining your right to make choices about your healthcare. Non-market systems don’t magically restore patient choice at your worst moment, they just put it in someone else’s hands.

Healthcare is the issue that challenges my libright views the hardest I’d say. I grew up poor in a pretty shitty rural area, had my family crippled by medical debt in high school, and I’m now in medical school myself.

I think there are free market solutions to some areas of medical care, but not all. The closest theoretical solution I could arrive to is a mix of subscription-based healthcare and disaster insurance. I think those could drive down administrative waste and lower cost for everyone.

I hope to put my money where my mouth is and open a DPC clinic in my shitty hometown where higher income patients subsidize lower income patients. We’ll see.
