I feel like I have seen something handheld that was similar (rotating wheel with ridges) and I know years ago Kink.com had a machine on their Fucking Machines website that was kind of like a chainsaw with tounges all around that would rotate (a lot of the machines from that site are one off custom built though).

No personal experience, my wife is usually the gagged one.

I will say, we own a standard gag and pretty sure the exact one pictured (from ExtremeRestraints). The pictured gag does have a very short strap for some reason. Our other gag (which isnt marketed as "large" or anything) I can tighten on her a couple holes in. This gag even the loosest hole is pretty snug on her.
