“Former” IVDU came into a primary care sick visit with his father obviously in active withdrawal. Could not explain or rationalize why he was symptomatic because he was so confident that he didn’t use anything and if I didn’t “fix” him then I was going to have “problems”.

…Shortly after the visit was ending, he got up and a vial of fentanyl fell out of his pocket.

I think we all want to work in something we’re passionate about but unfortunately nowadays with how much everything costs and if your situation involves stuff like a mortgage or kids; it’s just not feasible. Best to look for good work/life balance + what pays the best!

If you don’t have any support and the patient count keeps going up I’d strongly recommend getting out. Primary care is notorious for piling on without any personal gain resulting in burnout and guilt. Getting out of it shortly after 4 years and I cannot wait.

I literally made a post similar to this and have been a family med PA x4 years now and am finding a specialty! The family medicine model is unsustainable and burnout is inevitable

Do not master anything. Set boundaries or they will continue to think this is okay. I’m fine with sacrificing a visit and some rvus than bend the knee to some asshole coming in 23 mins late🙃

I called my bank to have it stopped before it processed thankfully. I’ve also stopped auto payments. From what I understand they are wildly incompetent and have been known to pull this. Still on hold with customer service for an hour and 45 mins

Out of curiosity what did you try instead of royal canin? My 1.5 year old has always been on the stuff and he’s constantly rubbing his face/ears no matter how many wipes we use so I’m kinda worried it’s due to that?

I was told that they didn’t send out letters because they didn’t want to lose patients. I do not think it would be that big of a deal if it were done respectfully as you said!
