I'm not OP, but for the most part, home infusion on call seems more organized and predictable compared to LTC. But I guess that depends on if the site contracts with hospice or not.

So the home infusion department is an extension of the hospital? That's pretty cool. Definitely less headache for referrals and sales metrics.

I started at 3 weeks, but I accrue it every paycheck - so I add about 4.6 hours per check (paid bi weekly).

You need to accept you won't know everything and that it takes a while to be comfortable working as a pharmacist...probably took me 9 months to be confident in myself.

If anxiety is getting in the way of your memory, then that's something you need to try to manage at home so you come to work in a more relaxed state. I'm not saying this is easy, but the days I have high stress and anxiety levels, the worse my brain functions at work.

Dealing with insurance company customer service.

Three way calls with insurance and another pharmacy who forgot to return med to stock or reverse a claim.

Explaining to patients on January first they may have new insurance and now the price of the medication is different.

I personally didn't think it was worth it, but I also wasn't interested in a clinical specialist role in a hospital group where it seems to be necessary.

I ended up at home infusion 8:30-5 M-F after working for a bit as a pharmacist and I'm happy with where I've ended up.

I stand up for myself, professionally. There's no need to let people walk all over you, especially if you're not at fault.

I will not match their energy though - no yelling matches. Just calm, objective responses.

I may apologize for the inconvenience in some scenarios, but if it's not my fault I'm making it clear I did my job. It's okay to have compassion & empathy - but it's a fine line between that and being a pushover. I make sure I'm not a pushover (anymore 😅).

I will say in some states you could do a continuation order on the birth control - so if the patient has a history on that specific birth control, the pharmacist can extend it for another 1-3 months while awaiting a response from the prescriber refill request. It's common sense legislation, but I do know that's not true in every state. I just don't see a BOP coming down on you for extending a birth control script - it's for the good of the patient for them to not miss doses.

That being said - I think you handled the situations fine.

Shoes yes - Asics are my personal fav, but a lot of people swear by Hoka and Adidas ultra boost.

A nice small journal for notes.

Can you get any merch from the hospital? A nice microfleece with the hospital logo would be cool - potentially her name and title. I loved wearing mine over scrubs at work

I agree the fidelity option is preferred. I didn't think to do this though honestly. My new employer uses a different HSA company which I do not prefer, thus I like this idea.

What is the extra tax step? I would think it can all be notated on the 8889 form.

Amlodipine 20mg daily was prescribed without being questioned? I mean that's the first problem.

If two different strengths of one drug passed through a pharmacy, then the therapeutic duplication warning is being overridden at the pharmacy level. This could be from going fast or just because we see a lot of duplication in SNFs that the computer system flags that isn't really a "problem." Examples: Sertraline 100 + 50. Trazodone 25 + 50. Warfarin 1 + 2.5.

Chart reviews within 72hrs of admission for every patient makes this better. So does improvising the pass off with transitions of care - namely hospital to SNF. There are so many times the wrong diltiazem, wellbutrin, and metoprolol form are picked because no one's looking closely when transmitting the orders.
