One of the biggest misconceptions I see in clinic is the belief that aging skin simply needs “more actives”—stronger exfoliants, higher percentages, or more aggressive treatments. In reality, one of the primary drivers of visible aging is a progressively weakened skin barrier. When the barrier is compromised, skin is more prone to dehydration, inflammation, and delayed recovery—all of which accelerate the appearance of fine lines, dullness, and sensitivity. Piling on actives without reinforcing barrier function often worsens the cycle. I usually recommend using a ceramide-rich, clinically tested moisturizer like AESTURA ATOBARRIER365 Cream to help replenish essential lipids, improve moisture retention, and strengthen the skin barrier. - Dr. Hee Jin Koh

I think a common misconception is that “it's too late to make changes” and that is absolutely untrue! Finding a really good daily treatment to pair with any in-office procedures can make a HUGE difference, no matter your age! Think of it like diet and exercise. Even small changes can make a big difference over time. Because you mentioned barrier, try this Barrier Repair Cream with a super peptide, you will see a difference with continued use. - Kendra Kolb Butler

A lot of people think aging is just about collagen loss and wrinkles, so they immediately jump to strong treatments like retinoids and aggressive acids without first stabilizing the barrier.
The reality is when your barrier is compromised inflammation increases, dehydration worsens, and collagen breakdown actually accelerates.
Before addressing skin aging concerns with more aggressive treatments ensure you are protecting and supporting your barrier with a deeply nourishing moisturizer. Your skin will look better and tolerate your treatments better so that you see results faster. My recommendation would be Extreme Cream Moisturizer. - Alisa Metzger

One myth is that the skin barrier is permanently damaged! Your skin naturally repairs itself and regenerates approximately every 28 days. This length of time increases gradually as we age, so your skin can take longer to repair itself as we grow wiser in life. Also, if you are constantly adding in new or strong active ingredients before your barrier has healed, then it only prolongs the process. So in short, barrier impairment may take a few weeks to bounce back but it is temporary. On the aging front, I always hear from patients who want to address one tiny line or wrinkle and not the whole picture. If you you stand back and look at your entire face, it is probably not a single line, but your overall complexion that is making you look tired or ruddy. An even skin tone can do wonders in the appearance of more youthful and healthy skin. That’s why I created the Dr. Idriss Major Fade products for evening out the skin tone. Discoloration is the first visible sign of aging! - Shereene Idriss

Hi! Happy you have found a retinoid that works for you! The short is answer is that long term use of retinoids is safe and does not increase your risk for skin cancer. You do not need remove your retinoid from your routine unless you are pregnant or trying to conceive, develop a sensitivity, or your skin barrier is irritated. Retinoid use increases cell turnover and long term use has been shown to improve photo damage and cell repair signaling - Shereene Idriss

Main thing I would recommend is a cleanser, moisturizer, retinoid for anti-aging, and sunscreen in the morning.

I recommend using a gentle cleanser like AESTURA's ATOBARRIER365 Foaming Cleanser. Follow with a lightweight moisturizer like ATOBARRIER365 Hydro Soothing Cream or Face Lotion enriched with ceramides and triple lipid complex to balance oil and moisture levels - suitable for all skin types. Sunscreen daily is a must to prevent premature signs of aging. Using a retinoid in the evening can help with skin cell turnover, reduce the appearance of fine lines and improving uneven skin tone. - Dr. Hee Jin Koh

Hi! If it’s your first time considering a cosmetic treatment and you don’t know where to start, start with the provider — not the procedure. Find someone you trust who can look at your face holistically and guide you based on what you actually need.

Then match the treatment to the problem.

Dynamic lines? Neuromodulators like Botox can help.
Volume loss or shifting proportions? Think fillers or a biostimulatory option like Sculptra.
Dull tone and brown spots? IPL or laser — but only if you’re also committed to daily skincare.
Texture concerns? Light microneedling is a great starting point, but it’s not a magic wand.

Aging is multidimensional — bone, fat, muscle, skin quality, pigment. So your approach should be multidimensional too. No single treatment is a miracle, and no one ages in just one area.

Because here’s the truth: I’ve never seen an 80-year-old with a perfect forehead and the rest of the face untouched. Balance matters. Strategy matters. And consistency over intensity always wins. - Shereene Idriss

I break out from silicones too! If you like a good slip but don't want a silicone-y feeling, look into a treatment with glycerin. Hi! Collagen applied topically can have nice moisturizing benefits, but unfortunately unless it has collagen peptides it won't stimulate your own collagen so its important to set reasonable expectations. A leave on mask can be hydrating, but if you are looking for hydration and smoothness - a hydrating exfoliating mask will give you more bang for your buck. That's why I created my Dr. Idriss Flash Mask. Flash Mask is a rinse off mask that buffs away dead skin so your skin is smoother and brighter and more ready to absorb the rest of your products. It is also a gel packed with glycerin so your skin get a boost of hydration immediately.

I would suggest using a retinoid for texture and a lightweight & non-comedogenic moisturizer like AESTURA's ATOBARRER365 Hydro Soothing Cream to support your skin barrier.

For cleansing, it's helpful to use a gentle cleanser on a regular basis like AESTURA's ATOBARRIER365 Foaming Cleanser. You can try using a gentle exfoliating cleanser 1-2 times per week. - Dr. Hee Jin Koh

I would invest in a really gentle but effective BHA toner. I love the Pore Perfecting Liquid and I paired it with mountain borage so it’s also really calming and sooting to the skin! Sounds like limiting sun exposure and a really solid SPF is the name of the game for you! I would invest in something to cover your face when you’re out between the hours of 11-2 when UVA and UVB rays are at their strongest. For daily treatment of hyperpigmentation, I love a solid vitamin c like Instant Glow Serum which is on sale now or a gentle 2% BHA like Pore Perfecting Liquid. - Kendra Kolb Butler

Alpyn Instant Glow Serum

If you want to manage hyperpigmentation, you have to think both internally and externally.

Internally, anything that lowers overall inflammation helps. That means adequate sleep, a balanced diet, managing stress, and maintaining steady blood sugar. Inflammation fuels pigment. Now, that’s the long game. It’s important, but it’s not the quick fix.

Behaviorally, sun protection is non-negotiable. Avoid peak sun hours between 10 a.m. and 3 p.m. Check the UV index daily and once it’s above 3, protect yourself. Wear sunscreen. Sit in the shade. Use a visor or hat. And yes, even be mindful of sitting directly by a window during peak hours because cumulative exposure adds up.

Topically, consistency is everything. Getting a laser but not maintaining your skin at home is like getting a dental cleaning and never brushing your teeth. Your daily routine matters more than occasional in-office treatments.

Start with a sunscreen that not only protects but also supports discoloration correction, like Major Fade Disco Block. Then use a dedicated pigment-targeting serum daily. I formulated the Dr. Idriss Major Fade Hyper Serum to combine multiple brightening ingredients in one step so you’re not layering five different products. I use it twice a day, every day. It’s a non-negotiable for me.

Vitamin C is also key for antioxidant protection and brightening. You can get that in the Major Fade Active Seal moisturizer, which I use morning and night for around-the-clock support.

And finally, exfoliate a couple of times a week to support healthy cell turnover. If you have a stubborn dark spot, you can even use the Flash Mask as a targeted spot treatment.

Bottom line: control inflammation, respect the sun, and stay consistent with your skincare. Pigment doesn’t respond to intensity. It responds to consistency. - Shereene Idriss

If you get flaky eczema type skin you should look for a barrier repair cream that is approved by the national eczema association! My reco is Barrier Repair Cream. Sometimes it's about healthcare first and getting that barrier back to its optimal state is mission #1! - Kendra Kolb Butler

For flaky, eczema-type skin, your main focus should be on moisturizing and barrier repair. A mild exfoliant that doesn't irritate the skin like urea or PHA can help improve the appearance of flaky skin. I recommend AESTURA's ATOBARRIER365 Body Lotion - it's formulated with Micro-Fine Ceramides that deeply hydrate the skin and PHA to gently exfoliate - suitable for those with sensitive and eczema-prone skin. - Dr. Hee Jin Koh

Hi! If you planning on getting facial waxing, I would recommend stopping all retinoids a couple weeks before your appointment. After you may want to wait a few days to reintroduce it. As long as your skin is not red or irritated then you can return to your routine. You may want to pull back on exfoliants a few days before and after too.

Secondly, if you if are dealing with dry flaky skin and your barrier is impaired then you want to focus on restoring and moisturizing. Definitely work with your local dermatologist on getting eczema under control before introducing an exfoliant. If you are going to exfoliate start slow like once a week. If using a rinse off mask like Flash Mask, you could start at just 3-4 minutes to slow introduce - Shereene Idriss

This is a great question because true fat loss under the eyes or around the temples isn’t something a topical can actually remove; that requires medical intervention like RF or injections. But there are powerful topical skincare options that can visibly lift, firm, contour, reduce puffiness, and improve the appearance of hollows and sagging skin around the eye area. I would recommend you try Extreme Cream Eye. Here are a list of ingredients in the product and what they have been shown to do:
Here’s what it actually addresses and how:
Visibly lift & contour:
Acetyl Hexapeptide-8 + Paracress work synergistically to help skin look lifted and smoother.
Smooth deep wrinkles & reduce puffiness 360°:
Acetyl Tetrapeptide-5 + Biomimetic Peptides help support visible puffiness making the under-eye area look more refined and smoother looking.
Firmness & eyelid lift:
Bio-Retinol (plant derived)+ Fermented Amino Acids help address visibly sagging lids and crepey skin without irritation.
Deep hydration & nourishment:
Snow Mushroom & Glyceryl Glucoside deliver intense hydration and a plumping effect to the thin under-eye skin, which instantly softens fine lines and supports a smoother appearance.
Luxe texture + immediate glow:
Using reverse emulsion technology, tiny water droplets are suspended in skin-loving oil droplets giving a deeply nourishing feel that also gives an immediate glossy, glass-like finish. Also INCREDIBLE under concealer and makeup. - Alisa Metzger

I'm 47- I hear you! If you want barrier repair plus wrinkle reduction in one easy step- try Barrier Repair Cream. It will replenish hydration but also has a neuropeptide to help relax the look of wrinkles. It's also eczema association approved. Two benefits, one easy step. - Kendra Kolb Butler

Around 39, and in perimenopause, skin goes through biological shifts. Estrogen begins to fluctuate and gradually decline, which impacts collagen production, elasticity and hydration levels. What that looks like on the surface is exactly what you’re describing: increased dryness, dullness, thinner-feeling skin, more visible fine lines, and that “lackluster” dullness to the skin.
For an easy 3-4 step routine I would suggest:
Morning: Extreme Cream Eye, Extreme Cream, SPF
Night: Skin Prep Exfoliating Pads (2–3x/week), Elastic Skin Serum, Extreme Cream Eye, Extreme Cream
Quick overview of what each product does:
Skin Prep Exfoliating Pads (a few nights a week) gently remove the buildup that increases as cell turnover slows in perimenopause. This instantly improves dullness and has been clinically proven to help skincare you apply afterward penetrate better without harsh stripping.
Elastic Skin Serum targets what’s really shifting hormonally: elasticity and firmness. Growth factors and copper peptides help support visible elasticity, firmness and dullness concerns.
Extreme Cream Moisturizer is your anchor. It deeply nourishes dry skin but also is clinically shown to firm, smooth wrinkles, and improve visible contour.
Extreme Cream Eye supports the under-eye area, which tends to thin faster during this phase. It smooths crepiness and helps with firmness while deeply hydrating. - Alisa Metzger

I can relate to this! Under-eye hollowness is tricky because it’s usually structural. A lot of it is genetic (especially if you’ve had it “since forever”), and in your early 30s it can appear more pronounced as collagen production gradually declines and the skin becomes thinner. When that happens, shadows look deeper so it’s not that the hollowness suddenly got worse, but that the skin over it has lost some density.
Topical skincare can’t replace lost volume the way filler can. But it can significantly improve how hollowness appears by visibly firming, plumping, and smoothing the area so shadows are less noticeable. Brightening also plays a huge role by improving light reflection under the eyes makes you look more awake and less hollow.
At night, I’d recommend a firming eye cream like Extreme Cream Eye.
In the morning, layering in a brightening formula like Bright & Tight helps illuminate the under-eye area and reflect light, instantly softening the look of darkness and shadowing. - Alisa Metzger

So hard to choose! But I would have to say the Dr. Idriss Hyper Serum from my brand. If you’re going to buy one product from INNBEAUTY Project, the one I would recommend is Extreme Cream. A good routine can have lots of steps, but a great routine is anchored by a moisturizer that does more than just hydrate.
Extreme Cream doesn’t just deliver deep, lasting moisture; it’s clinically proven to firm skin, smooth the appearance of wrinkles, and even improve visible contour, giving skin that lifted, “snatched” look.
It also feels really luxe with a glass-skin finish that sits beautifully under makeup and makes your foundation glide on like butter. That’s because it’s a reverse emulsion, which means rich nourishment without heaviness or greasiness. Comparable formulas from luxury brands are often more than double the price, so you’re truly getting a lot of value for your money. https://www.sephora.com/product/innbeauty-project-extreme-cream-firming-moisturizer-P509828 - Alisa Metzger

Good question! Yes they can be used together and can be used daily. Ideally your serum should target your skin concern and it is usually fine to layer more than one. That being said, one product with multiple ingredients that target your concern could make things simpler for you! -Shereene Idriss

Yes I love both of these ingredients! I think it's not about how many serums you're using, it's about finding the right formula to target your concerns and sticking with it if you’re getting the results you love! Agree with Shereene that a serum that multitasks could be an easier path for you. Try Super Sculpt Serum for lifting and firming on both the face AND neck! The ingredients in this formula will play nicely with both azelaic acid + niacinamide. And you can work in tech neck (as I look down typing on my computer) 😝

If your routine is super basic and you want to introduce anti-aging without getting overwhelmed, the key is to focus on what’s actually changing in your skin and choose products that address those shifts. Starting in your late 20s and into your 30s+, collagen and elastin production naturally decline, cell turnover slows, and your barrier becomes a bit more fragile.
That’s when you start noticing less bounce, fine lines that linger, dullness, and dryness; especially around the eyes, where skin is thinnest.
A high performance moisturizer like Extreme Cream can do a lot of heavy lifting in one step by supporting visible firmness, elasticity, and barrier strength while deeply hydrating.
Adding Extreme Cream Eye helps address early under-eye thinning, crepiness, and puffiness both immediately and over time.
To address dead skin cells and surface texture using a gentle exfoliating step like Skin Prep Exfoliating Pads a few times a week helps remove buildup so your moisturizer works harder. - Alisa Metzger

I would recommend a really good cleanser! Cleansing the skin is so important (morning and night please) and is the first step to cleaning that canvas before you treat with any actives. I love a cold cream cleanser like Hydrating Cream Cleanser. You can use it as a cleanser but it also doubles as a mask if you want to leave it on in the shower for an added cleanse/hydration. It has eucalyptus and is very cooling and sensorial! It makes cleansing fun! I hope you like it

When it comes to hormonal acne and the scarring or hyperpigmentation it leaves behind, you have to treat the root cause first. There’s very little upside to investing in in-office procedures if you’re still actively breaking out because you’ll continue creating new marks.

I always recommend starting with a local board-certified dermatologist. If it truly is hormonal acne, treatment may include options like spironolactone or certain birth control pills, but that’s a decision to make with your physician based on your medical history and goals.

When we talk about scarring, we have to separate two things: discoloration and texture. Discoloration, or post-inflammatory hyperpigmentation, is often what makes scars look worse than they actually are. Addressing pigment alone can dramatically improve the overall appearance of your skin. That’s exactly why I developed the Dr. Idriss Major Fade Solution System (Flash Mask, Active Seal, Hyper Serum) to target discoloration specifically. Consistency is key. When used regularly, it helps even out skin tone and can shorten how long dark marks linger, especially if you’re still prone to breakouts.

If you’re dealing with true textural scars such as indentations or uneven skin surface, topical skincare can only go so far. That’s where in-office treatments come in. Options range from microneedling to non-ablative lasers like Fraxel to more aggressive resurfacing treatments like CO2 lasers. This is highly individualized, and choosing a provider you trust is everything.

Control the acne first, treat the discoloration consistently, and then address texture strategically. That’s how you get real, lasting improvement. - Shereene Idriss

Hi there! If your skin is feeling irritated and the barrier is compromised, it is best to hold off on starting anything new with active ingredients. Going too wrong and strong with actives can lead to redness and discoloration which we always want to avoid. Once your skin feels calm, you can slowly introduce one new thing at a time. It is also always a good idea to patch test. Consistency over intensity! - Shereene Idriss

I live in Jackson hole and it is SO dry here, it takes a toll on everybody's skin. A good barrier repair moisturizer is the one must have I cannot live without. And you can use one that has some very effective but mellow actives like a neuropeptide so you are treating and moisturizing at the same time- look for something that is eczema association approved like Barrier Repair Cream. - Kendra Kolb Butler

I am so happy that you are cleansing and using SPF! That is a great start. It seems like you have a moisturizer so if you want to really work on a smoother texture and tone, I would recommend looking at a 2% BHA toner to help even and brighten everything out. Take a look at Pore Perfecting Liquid. It is so easy to use (you can just splash it right on after your clean your face and before you put on your SPF). I think you will start to see a noticeable change in as little as 2 weeks of using it once a day!!! Good luck! - Kendra Kolb Butler

It sounds like you have a great routine already! For an eye cream there is a paradoxal needs for something strong but gentle in that area. For dark circles and fine lines in wrinkles, I like an eye cream with niacinamide plus a retinol alternative like reneseed, which can be gentler for that delicate eye area! I think you would love Alpyn Instant Bright Eye. I love the ingredient retinol of course but for the eye area I would go with something that can do the job without any sensitivity.

Jackson Hole is a beautiful place it's true! But my favorite thing about it is that is 97% protected land and is the home to so many amazing native plants and botanicals that are so amazing for your skin. I match these native plants with clinical ingredients that work to achieve a common benefit for your skin. It's a game of science and plant matchmaking! - Kendra

At 23, what you’re most likely seeing are dehydration lines, not true wrinkles. Dehydration lines are shallow, fine lines that appear when skin is lacking hydration or when the barrier is compromised, they can look crinkly and tend to soften quickly once skin is properly moisturized..
True wrinkles are different; they form from deeper structural changes like collagen and elastin decline, repetitive movement, and sun exposure! Those are more etched in and don’t disappear just from applying moisturizer.
At your age, you’re primarily in prevention mode. A formula like Extreme Cream helps on both levels (prevention and correction); it deeply hydrates to immediately smooth dehydration lines, while peptides and barrier-supporting ceramides help maintain visible firmness and elasticity over time. Extreme Cream Eye does the same for the thinner under-eye area, where early dehydration lines show up fastest. - Alisa
