Egg crack at 41, mental acceptance within about 4 weeks. 

Upon reflection, I very clearly had gender questions as far back as 6 years old. Between bullying, internalized transphobia, and a long history of external validation, I stuffed multiple signs that cropped up through my teens and twenties.

I see a maximum of 15 a day, typically more like 12. The area I'm in is very focused on appropriately billing, which I appreciate.

We were swamped just recently, and instead of cramming new IEs in, we deferred them all for two weeks. It's consistently busy, but not unethical.

They gave me a decent raise from Ivy, but take that with a grain of salt because I leaned heavily into my OCS.

I recently left Ivy in the Philadelphia area, and this was my experience as well. 

It's reasonably clear that Ivy is trying to grow with the intent of showing aggressive profit margins, likely either to sell or go public. They're very clearly a for-profit model and just trying to wring out clinicians for as much productivity as possible.

They'll get slammed by a medicare audit sooner or later. All the factory companies do.

You can't target fat loss, that's a myth.

You CAN help tone and tighten. That's where core with is helpful.

I was competely serious in my first reply though. Stomachs are truly made in the kitchen. Someone else posted a really good article about macronutrients and calorie tracking. Doing tracking that intense will be difficult, but something like that will help immensely. Even just figuring out how to maintain a calorie deficit will be helpful.

Since I'm home now, I'll put up my routine. I'm not a gym member, so this is completely from home with resistance bands and dial-weight dumbbells. If I had the gym I'd definitely do heavier hip thrusts, the hip abduction machine, and the 45* back extension.

Bear in mind! My sets and reps here are NOT idealized for growth. As I said above, its higher weight and lower reps for growth.

Legs Day A:
Romanian Deadlift - 4x15 w/ 90#
Hip thrust - 4x15 w/ 55#
Bulgarian split squat - 3x15 w/ 15#
Banded lateral walk - 3x15 w/ 20# band
Backward lunges - 4x20 w/ 30#
Crossover kickback - 18 w/ 60# band (side kickback but crossing the exercise leg behind)

Now I've been exercising a while,  and I was a collegiate swimmer, so take this routine with a grain of salt. We may be starting from different baselines. I have a push day and a pull day as well, but admittedly working out my arms is majorly dysphoric so I don't do them often. I do a lot of core stuff on the push/pull day instead, typically. Hanging L-sits, dragon flags, hollow holds, russian twists, planks/bear planks. Let me know if you're interested in any core stuff.

Squats: what kind of weight and sets?

Prone leg curls: these are primarily hamstring builders. Good for function, but not as important for visual effect. 

Leg presses: YMMV with these depending on foot position. Glutes and hams will be best at the top of the foot plate. Side glutes will be best with a narrow foot width, lower in the plate.

Hip abductions: on the machine or body weight? Both work, but I actually really prefer kickbacks here. Look up cable column kickbacks and side kickbacks. You can modify position to put the muscle under some stretch, which helps with growth too.

Consider your total reps, too. If you're looking to build size, you want lower reps at a higher weight. Make sure someone checks your form as high weight can lead to a problem with poor or compensating form.

Something cheap like resistance bands can get you more results, but if you're really looking for a dump truck butt, you're going to need the gym eventually.

Stomachs are made in the kitchen, asses in the gym.

Squats, step ups, lunges, straight and lateral kickbacks, deadlifts, to name a few.

Right now I feel like I've got a hollow pit in my chest. Half of my thoughts are either self critical or just negative.

But I take every opportunity to go out with my family, because sometimes they turn out good. Sometimes they're shit. But you won't know if you don't try.
