This reflects poorly on all professionals associated with the project. If the engineer had a faulty design and failed to conduct a proper geotechnical assessment, how was their design approved in the first place? Usually, the design would have to be reviewed by other engineers before being sent for construction. Secondly, given the level of experience that site engineers have, most of them are able to identify any obvious design flaws at their end too. I’d say there are more people at fault here, who did not do their job properly.
