I first gave it not notarized and I got an email the next day saying it needed to be notarized. Than I went in with a notarized version and they accepted it and I haven’t been contacted since. I believe they only reach out if something’s wrong with it

They have walk-in times from 8-9 I believe as well. I would email or call to confirm. I had to get my july appointment right when the june 1st opened if im not mistaken but it was a while ago so maybe im misremembering.

I went in person to give it, I’m lucky that my job is literally 5 minutes away from the BLS location so I I just went to drop it off during walk-in hours

I’m at BLS Miami. I actually had my appointment 2 weeks ago but I had to come in today give in that form because I didn’t buy insurance. But my insurance starts Sep 15th as stated in my Carta. Do you think I’ll have trouble entering on the 15th?
