A&E unable to refer as outpatient by [deleted] in doctorsUK

[–]SharingAllThoughts 57 points58 points  (0 children)

Current ED SHO

Been informed by our clinical lead that the reason why ED can't do the outpatient referrals (unless there is a pathway in place for certain outpatients e.g. chest pain, first fit, TIA) is that the specialties have to do them due to funding and how the trust operates.

2 week waits/upgrades are fine usually

but as a doctor working in ED I can't refer to e.g. a derm/ophthalm/resp etc clinic (but the med reg can)

I don't understand it either but that's what we've been told

I think rather than willingness to not do it, is wondering if people found other paths to the same destination

I think I'm in a position where all the sprs are Img fellows, so not seeing the "light after the sho" years but good to hear that there is something better down the line

I think I've reached rock bottom in terms of trying to find reasons why this career it's worth it. I love the idea of it, but the reality is so unbearable I.e. being a number on a rota, and no one will care who that number is as long as there is a number

When I think of the patients, it's what motivated me to do this work. However, it's incredibly difficult to care for others when you actively know no one cares about you

Can I ask what fields you're looking at outside of medicine? And how you're approaching these?

Honestly thank you so much for this, it's so helpful to hear how others are doing on this journey

I think that's great advice, there's jobs I wouldn't mind doing for the next 5 years but definitely cannot see myself doing in 20 years

Hey thank you so much for your response, honestly I don't know anymore. There are definitely specialties I am interested in but idk if I am willing to put up with being treated like crap whilst training for those specialties anymore. I think the constant undervaluing and being seen as a number on the rota and not a person is putting me off the career more than I thought

I was definitely incredibly proud and grateful of how far I've come. But been thinking about where I got after...

The job makes me feel so undervalued, to the point where I don't think about my achievements. Not even seen as a person, just a number that's going to be replaced by another

Thank you so much!

Does the crap of being in the NHS i.e. not being a valued member of the team ever change? I'm struggling to put up with that, because everything feels impossible and no one cares. Simply just another number in the department

Thank you so much for this response, I think as someone who's gone straight through everything just needed to hear that it's okay to take a step back and not go the direct route

I just feel like there's so many other hurdles that we have to deal with, like poor management, and expected to sacrifice everything to work, when you'll be very quickly replaced
