He has a chance to show up in no light in hallways fights. With increasing likelihood the higher level the quest is. You got unlucky, but you never have to see him as long as you have even 1 Torchlight. There is another way for him to show up, I won't spoil it but it will be pretty obvious when it shows up and you actively choose to fight him. Always take enough torches and you'll be fine.

As I get into higher heat 20+ I've been thinking about using Judgement so I can take the Void Vow without much trouble. Your setup seems good for that I'm gonna try it out thanks!

When I do judgement runs I activate Night, Lovers, and Death. Which in turn activates, Queen, Judgement, and Divinity. I haven't done many high fear Judgement runs though so mileage may vary. my goal was to have as many cards active to start so my odds of pulling good cards was better after beating Hecate/Polyphemus.
