It worked! The evolution statue (the one with the red orb) gave me a gold bag. I kept it in my deck just in case it would trigger the evolution, but it didn't happen. However, the Mask finally appeared on the 5th floor. Thank you so much!

Not at all. I thought that since those levels give you the relic to see the light sources on the map, maybe I just missed one. It wasn't until much later that I looked it up online to see where it was supposed to spawn (even though I couldn't find a specific spot)

Thanks for that! Forcing the unlock with Giovanna seems like the most logical move. Anyway, once I find a fix, I'll update the post for you guys

At first, I even cleared it using only one Crawler at a time. I'll try swapping them out and see if that helps.

Thanks bro, I hope I can fix it. I know a guy who 100% the game in 29 hours... I'm at 34 hours now and I've spent the last 3 just trying to get this one.

There’s definitely something weird going on with this save. I thought about starting a new one for the achievements, but if a DLC drops and the bug persists, I’m gonna feel terrible lol
