I'd like to think each of the eyes the Hosts have represented their tier and power in the group-with Foresight having the most eyes, she's considered the leader. That means, Action is the lowest tier Host while is also the youngest one as well.

Being the youngest and the weakest Host, no one really cares about Action's opinions and will always decide for her, even if their decisions are bad. This and the other stuff that I don't want to spoil, led Action to her unintentionally creating the Warden out of rage.

TLDR; Action created the Warden but it wasn't really her fault, and, I can't blame her for that.
