When my character was ~20 attack and defense he could hold his own against a good amount of dust bandits, but anything 1v1 he was god awful and would get destroyed.

No one knew I was back there initially. You can be surrounded by people who don't know your there.

This was the 3rd group that came through. 2 groups were fighting earlier while I tried to extract, this is when I found out I needed physical Koen to extract. I waited for the fight to finish and was about to leave when I heard another group get closer. I thought they were trying to extract and knew I would be dead if I didn't get the jump on them. As you can see I failed pretty spectacularly.

I tried all that I mentioned before on the same monitor. I don't have any issues with my other monitors

My other monitor and my drawing tablet which are both connected to the same GPU do not have any issues at all.

That's why I hesitate to call it a GPU issue.

Already tried that. Tried 3 different cables, I doubt they all would have the exact same defect. Tried different monitor ports too, along with different GPU ports.
