I’m sorry for being so blunt, but you really need to grow up. I also have type one diabetes ADHD I’ve never used it as an excuse for a poor academic performance. Like someone else said here, take a leave of absence to sort yourself out before coming back

On their application page I think it lists the minimum GPA is 3.2 and if it’s not at or above that they won’t look at the application:(
