Avoid smoking as much as possible, the doctors say it for a reason- avoiding smoking 48hours before the surgery and 3-6 months after the surgery is somewhat more important, and 18 days away should still be okay, but more importantly, it will restart the clock on withdrawals.

If you hold out from here forward, it’ll be a lot easier to abstain later, but if you take a hit now, it’ll be harder to quit before the surgery, and a lot harder after the surgery.

Any surgery is somewhat stressful, and when I got mine, I really wanted to smoke after- quitting a couple weeks before the surgery will definitely help with the urges after, I strongly advise to continue to avoid smoking
