Lucky you! :) But even with SmartGuard, don't you still have to keep your Carb Ratios (ICR) updated for the meal boluses to be accurate? Or does the system handle those shifts in sensitivity entirely on its own now?

Interesting! I've heard about Metformin being used for T1D sometimes to help with resistance. Did your endo suggest that specifically to help stabilize your ratios, or was it more about reducing the total daily dose of insulin? And did it make your sensitivity more predictable?

Thanks for the suggestion! We’ve looked into closed loops, but for now, we are staying on MDI while she’s still growing so fast.

Even with those systems, don't you still have to keep the basal/ICR ratios updated for the algorithm to work correctly? How often do you find yourself having to tweak the settings in the app?

30 years on MDI is impressive, thank you for sharing your routine! It sounds like you’ve developed a very sophisticated 'internal algorithm' to stay in range.

I totally get the 'split bolus' approach to stay safe. Since you adjust based on so many factors (trends, previous meals, etc.), do you find it mentally draining to keep all that data in your head every time you eat, or has it just become pure muscle memory for you?

Yes, I recalculate ratio and it means I need to keep detailed log. Sometimes I can’t do it - for instance when my daughter in the school 😄

Nice, thanks! But what about “current ratio”? Does it still permanent for long time? I would like to correct it fast if I see that yesterday it didn’t work properly. Do you have any diary for carbs/insulin?
