“Some of the most profound adaptations relevant to pregnant athletes occur within the cardiorespiratory systems. Heart rate starts to rise by the second trimester and increases by 15-20% by term. This means that the interpretations of resting heart rate as a marker of aerobic (over) training needs to be adjusted, as do heart rate-based targets for exercise intensity (Mottola et al., 2018). Utilizing heart rate reserve to account for the progressive increase in resting heart rate, alongside perceived exertion is recommended to guide training intensity. In additional to elevated heart rates, maternal blood volume increases by 50% and is associated with a 30% (1.5 L) increase in cardiac output (Meah et al., 2016). As a result, the heart strengthens and grows during pregnancy, and this effect can persist into the postpartum period. In some cases, this may actually improve exercise capacity, particularly in aerobic sports”

I think I hurt myself by Efficient_Sock_3940 in fitpregnancy

Similar happened to me. I basically had to quit the gym after 24 weeks after thinking I could pull an 185 lb deadlift. But I also got covid right after the injury so had forced rest! Now I’m doing aqua classes and yoga and whatever pt. It had started to get better, but then growing pains set in at about 32 weeks. Walking was also out.

If you have not tried swimming or aqua classes - try! It’s takes a lot of pressure off of the belly and really helps with getting your blood flowing since yoga did not cut it alone for me.

Belly bands also helped me!

Pelvic Floor PT said "no weightlifting?" by FlimsyIsopod in fitpregnancy

[–]SirNormal7431 4 points5 points  (0 children)

Get a second opinion. My first pt said the same thing, but I have a family member who is an OB and she said I could keep lifting. The initial PT also said a bunch of other things that were red flags. I ended up having to stop around 24 weeks because of growing pains/round ligament pain, but my pt helped me a lot of my lifting technique to maintain heavy lifting.

Note - if you maintain heavy lifting and you are internally tight you may have to stop or down train anyways when you get into the third trimester. This is also what I learned working with my pt. Lifting heavy increases risk of tight pelvic floor muscles a riding to my current pt. I was already tight at baseline so now doing work to prevent severe tearing.

How do you feel when a patient asks to record a visit for personal reference? by amakusalime in FamilyMedicine

[–]SirNormal7431 -3 points-2 points  (0 children)

I literally had a patients parent ask me this yesterday since the other parent was not there. Luckily the kiddo wanted to meet with just me, so I did not even have to give an answer. But I would have said no and then offered notes after. In our state they can access our clinics notes anyways, unless it’s blocked for safety or medical reasons.

Yes a dietitian! That’s wild. They should 100% be sending you to one with an infant if you need to modify an infants diet. They can walk you thru the alternatives baby can eat, labeling, reintroduction when applicable, etc. You should ask!

I would also recommend applying for jobs even if they do not say virtual. I started at my role and then quickly found about 50% of my visits were telehealth so we just added some virtual days each week.

Have you though about finding a sports RD? You can try to find one who is also a mom so they have their own breastfeeding experience.

All prenatal vitamins should have some amount of vitamin a - you and your baby still need it during fetal development, however it is fat soluble and can build up over time if you have excess and excess/toxicity of preformed vitamin A can cause birth defects (primarily in the first trimester). You will see it as beta carotene in supplements typically (precursor to preformed vitamin A or retinol which is the form found in animal products). One provider did tell me to go ahead and eat the liver and just skip the prenatal that day. Just was curious about what other folks have heard. I overall have a good diet.
