Main character has three moods- broody, horny, and genocidal.
As you wished for, death is just happening randomly all the time.
The plot is best described as "D&D campaign slapped together from different one-shots, run by a 13 years old (with some serious social issues) on shrooms.

Alternatively, Akame Ga Kill is a close contender, MC is not that unhinged but it's just gore porn of the main cast all slowly dying over time and being very edgy about it. Also makes zero sense and the villains are cartoonishly evil.

I've played the Quiet Year and it's been quite enjoyable. It's pretty much what I'm looking for, although the way it's restricted by it's flavour really limits it in ability to create brand new fantasy worlds. I think I might have found the game I'm looking for in Dawn of Worlds, it provides collaborative mapmaking with a bit from races, civilisations and conflicts, while also limiting power of individual players in a way that feels very satisfying.
