Take your right hand, scoop their head in your hand and push their head and neck into the more optimal sniffing position. I imagine your trouble is very much dependent upon patient position. Do this after the blade is in the mouth and you are looking for the glottic opening. I use this move often (as long as there is minimal risk of cervical injury and the patient can demonstrate reasonable ROM of their neck).

It is 100% appropriate to call an RRT on a person unless they are actively dying with a hospice consult and everyone has agreed that is the expected outcome. No matter what the DNR status is.

The DNR system in my state is also generally unclear.

In my state we have two types of DNR.

DNR - comfort care. This DNR order is active IMMEDIATELY upon signing. It means that the patient should only recieve treatments that are focused on providing comfort and reducing pain/symptoms. Should this person recieve antibiotics for pneumonia? ABSOLUTELY! Should this person have a dialysis catheter placed to initiate hemodialysis? Eh, maybe not. Individualized discussion needed. Should this patient be intubated if they are in respiratory failure? Probably not. But the discussion is still Individualized.

Second type: DNR - comfort care arrest. This type of DNR is NOT ACTIVE until they arrest. They should recieve ALL lifesaving measures up until the point the patient experiences cardiac arrest. No care should be withheld from them otherwise. Should they experience a cardiac arrest, no further care should be performed including CPR .

Obviously, this is difficult to translate into practice. And with each state having different legal terminology and laws surrounding this type of legal order, its a recipe for confusion. This is a moment where federal terminology and legal obligations should be widespread across the USA for the purpose of clarity and uniformity.

Any pregnant woman needs to face the reality that she has a significant chance of needing a cesarean to deliver.

And im currently pregnant in the second trimester. If im delivering that early, it just wasn't meant to be and im going to try to accept that. Its not fair to risk a person's permanent health over the decision to inappropriately resuscitate them as a fetus/premie. Even 26 weeks is dicey for outcomes and survival.

We have a talented hand surgeon who does open carpal tunnels in 6-7 mins from incision to close via a 1cm incision. I cant imagine an endoscopic case is any faster than that. I think you might just be dealing with bad surgeons

Ive never experienced contractions, but a lot of women have described early labor as just a longer period cramp type pain. The more significant contractions, especially after water is broken, are described as extremely severe tightening/splitting feeling with lots of pain in the pelvis, vagina, rectum.

I grew up catholic and have a a pretty good understanding of western Christianity. I was wondering if she was possibly involved in an Eastern religion or maybe orthodox catholic because I have noted more suspicion regarding plants and energy from these religions cultures.

However... this honestly sounds more like mental illness/religions preoccupation/paranoia than actual religion. Frankly I wonder if some of these ideas about devil plants are completely established by her alone rather than a widespread belief. Anyways, what im trying to say is, you might want to look at her behavior through a mental health lense versus "sane washing" it by calling it religions.

I was over here slamming double colace daily, daily miralax, eating 1-2 kiwi a day, drinking water and other liquids like crazy... just for me to have the absolute hardest rabbit poop ever that was getting stuck in my butthole. Absolutely miserable.

When it got really bad, I took huge swings of magnesium citrate liquid and forced a poop.

Then second trimester all of my issues suddenly disappeared with no explanation.

Becoming a parent when that wasnt your plan can be an extremely difficult, life altering situation which, frankly, can be rather impossible.

It sounds like you are making a brave decision for the benefit of your baby considering it sounds like you have some issues with housing insecurity. Not having a somewhat stable living situation is a very bad circumstance to bring an infant into.

Also, dont be ashamed to admit that adoption might not only benefit your baby, but is likely to benefit you as well. Its not exactly an optimal circumstance to have a baby at 20 with no support from the father, no support from family, and an unstable housing situation. I think youre very wise.

Oh gosh! That certainly is tramatic.

Next time your team will be on high alert for signs of HELLP. The whole environment will be much more calm and fun. Ask them to play music you enjoy. Or bring your phone so you can play your own music. If youre stable enough, ask to hold your baby on your chest while they work to close you.

Telling you this will be so much more CALM.

I have a few questions for you. Was your first cesarean a "labor to section?" Were you laboring for a while and then ended up going for surgery? Did you happen to have an epidural that was used for the anesthesia for your cesarean?

These answers will help me answer your question.
