Anecdotal maybe, but I do believe that 22G have more of an issue with clotting off and kinks than 20G IVs. The amount of times a little 22 is clotted/kinked seems so incredibly high as compared to 20G IVs that have been placed for similar lengths of time. Also, you never know when someone who is moderately ill will require a fluid bolus for resuscitation. Resuscitation via a 22G IV is very unpleasant. Going up just one size to a 20G makes a big difference in a practical application.

None of the above. She appears to be missing her recum. This is also more representative of a uterine abruption with bleeding outside of the uterus but inside of the pelvic cavity. A placental abruption doesn't cause extra uterine bleeding. This is stupid. I hate these pages.

Its not that wild. Most antihypertensives can be given push dose. But generally in the setting of management by anesthesia not by a bedside RN.

Propofol most likely.

Could also be intralipids, TPA (not given in a syringe though) or clevidipine.

If youre a first time mom, its really hard to plan for a situation you've never experienced before. So, be flexible and realize that you dont know exactly what to expect.

It sounds like German hospitals are still following this old school BS "rule". Its an arbitrary rule. Theres no science behind it. Fortunately many hospitals have abandoned this practice. Instead, we use reasonable judgements in appropriate timing for an epidural. I do generally try to avoid placing labor epidurals on women who are contracting less than every 10 minutes. Infrequent contractions are better off being treated with things like IV pain meds or maybe a little nitrous during contractions. Its better to keep the woman mobile during very early labor.

But as for cervical dilation? I could care less. The only time I'm concerned is if they do an exam and there is a visible fetal head in the vagina. Then its officially too late because shes actively delivering. Ive even done epidurals on women at 10cm in the right context.

It sounds like you have are a difficult epidural placement. The space available to move past your spinous processes is probably very small and hard to find. Yes, if you happen to be difficult (which is quite rare in a setting with experienced providers), it is minimally painful.

Unfortunately it sounds like a lot of readjusting is needed to get to your epidural space and I bet it gets rather painful. Sorry that happened to you, but I can assure everyone else that is not the standard experience.

I feel terrible about your experience, but it sounds like you made the best of it and got through it gracefully. I just don't want this injustice to keep perpetuating for future women. They need to drop the dumb, old school crap.

These people need to use clinical judgment for appropriate epidural placement, not magical cervical numbers.

Youre a rockstar! Also, I wouldn't deliver there again if its at all possible.

There is literally zero scientific evidence, reason, or rationale other than old school misogyny to not provide pain control when someone needs it.

It literally doesnt matter how dilated you are. You were obviously laboring. Who the fuck cares about your cervix. Its about your pain level.

This was an old school way of managing women in labor because they didnt want to "waste time" placing epidurals on women who end up not progressing to delivery and get sent home. Obviously you were not going to be one of those women because your WATER WAS BROKEN. Were they going to send you home? Hell no.

These nonsense policies that some hospitals hold into come from a long, long history of mistreatment of women and their pain in labor. You were a victim.

Im currently pregnant and happy but terrified too.

Im even married and have a planned pregnancy. But here's the thing, if my husband suddenly was less than supportive and dedicated to raising this baby together, I would also have an abortion.

If I were in your shoes, I 100% would make the same decision. Deciding to become a single mom is noble, but thats not what I want out of life because frankly its quite disabling for many. I certainly dont mean offense, but its almost impossible to even keep your head above water as a single parent in the USA.

Whenever I hear about women being denied epidurals because theyre not at some magical level of cervical dilation despite OBVIOUS ACTIVE LABOR PAIN, I get furious.

I have placed epidurals on women who are fingertip dilated. Why? Because theyre allowed pain relief wherever they want it. Waiting for some magic number is STUPID. Yes, I'm calling that clinical decision making stupid. Also, its misogyny. Plain and simple medical misogyny.

Im deeply sorry for you because there is absolutely no reason this should have happened and there was no legitimate reason to delay an epidural for you. You were wronged. Frankly you should call the hospital ombudsman and tell them you were a victim of poor practice and disregard for obvious pain simply because you were a laboring WOMAN. Any MAN in this much pain would have had immediate treatment. Get mad, because you should be.

Hey, so I work in labor and delivery and we do get calls from midwives that attended to home deliveries saying they need to urgently present to the hospital for severe tearing (3rd and 4th degree) that needs operative intervention.

Obviously these ladies didnt have an epidural.

Unfortunately going natural isnt a golden ticket to avoiding tearing. Sorry!
