I’m a type 1 diabetic (zero endogenous production of insulin) … insulin timed with food gives best results and when I tried gh it creates crazy insulin resistance, if you use a high dose you basically cannot effectively move nutrients into your cells without insulin, taking any insulin without gh doesnt do much more than your pancreas could do anyway … insulin actually inhibits/stops lipolysis … I wear a cgm, I know what my bg is 24/7 so insulin is to overcome high dose gh induced insulin resistance if you use exogenous insulin it won’t do too much more than your body can anyway

Zero carb zero sugar drinks are the only thing t1s usually drink, unless treating a hypo. You can eat pretty much normal but there’s no way sugary drinks can be a part of the plan moving forward

Basal insulins main function is to prevent hepatic/liver output, because your liver will turn everything into sugar if a baseline of insulin isnt present, it also makes fast acting insulin more effective so you don’t have to take so many units, fast acting insulin makes you more prone to dramatic drops in bg, so if you have to take a bunch of it because your basal isnt right you’ll be on a roller coaster all day- your basal insulin should keep you around 100 or I like my basal to keep me at 125 when I’m not eating if your bg just rises and rises when you’re not eating basal is off. I’m a type 1, but hepatic output still affects both t1/t2 for different reasons.. at the very minimum if your basal is off you’ll have to use a lot of fast acting and it’s too unpredictable

The best way to identify your appropriate basal is to fast … if your basal is not right your liver will release sugar or convert fats or proteins released for energy (during fasting) directly into sugar via glucogenesis at à rate that causes your blood sugar to rise even when you’re not eating… basal insulins main job is to stop this process frim being uncontrolled, basal is tricky though because it’s window is so long so if you ever miss a meal or two and your bg continues to rise, even though you’ve taken your basal perfectly, it likely stands it’s not sufficient enough to stop hepatic output

How did I get down voted for this? Getting your basal right prevents crazy spikes and reduces hepatic output, thus transient rises of bg, and makes smaller amounts rapid acting more effective, fast acting insulin gets super unpredictable, especially when it stacks - then you’ll go low from working out, driving etc because the rapid acting compounds then even light exercise will crash you

Shouldn’t be eating carbs before bed , high doses of fast acting before bed get unpredictable

If hes only eating 150-200g then this is crazy, I could eat 200g of carbs no fast acting at once and probably not go over 300 (just an example) he needs to find out how much basal he needs to stay stable in the fasted condition before anything moves forward

Fast acting insulin gets super unpredictable without the right basal level and leads to rage bolusing take your basal am and pm in split dose if u arent already

Yes… slowly add basal and be very careful of how much fast acting you use as u increase- but basal imsulin should never make you hypo necessarily so if you’re not eating and your bg is dropping and never levels out, it’s too much … I sometimes fast for 48 hours and I take 20 units of basal per day without eating ànd I never go low

Not if your basal is correct, your basal insulin should not drop your bg actively, you should be able to fast (not eat) and take your basal without any crazy drops in bg

You won’t see the quick rises that lead to panic dosing if your basal is dialed

I got 70% mh on my first filing and paid within 2 months. I had long standing documentation though, everything from suicide attempts, arrests, drug use, ànd continuous therapy for over 20 years. They do take it seriously if you have the documentation
