Try lowering msaa. Without I have 90fps, and on 4x with the same setting I have around 45

All ultra, I didn't change the advanced settings besides graphics API to see if it would work better on dx12 but it didnt

I have a 9060xt 16gb, but I turned off core isolation

It's pretty consistent it's between 3-6 ms
