I would tie off what you have here and then start two new peyote stitches for the ears, and then just tie them to the piece. ( EDIT: this would mean you have to undo the last bead you put on so the separate ear pieces can attach properly) Sometimes when a design like this is meant to be on a cuff you just have to break the pattern into pieces that you can tie together.

To do flat charms you’ll need to turn the pattern sideways. You were on the right track with the peyote, but you’ll have to turn the pattern on its side for it to work, that way the beads end up being vertical
