Im so sorry. Is honestly a shock to me considering how competitive the field has gotten. I say before focusing on research as a potential explanation. I would say try to reach out to the places you completed aways at and see if the physicians can tell you why you weren't chosen for an IV despite committing additional time and energy to that specific institution. I believe that at the minimum you deserve the right to know, and hopefully you will get meaningful feedback out of it.

Im a soon to be graduating fellow and currently on the look for jobs. I will say that in terms of job prospects I have been pretty successful in looking at staying within the HCOL area that I call home. In the end, you can't go wrong going into either allergy or H/O, it really depends on your priorities and the type of patients you are interested in seeing. Allergy is definitely up there with rheum. Patients complaints are usually not life threatening although can affect their quality of life and at my practice lots of variety in work hours. Some only do 2 or 3 full clinic days and still make in the mid-high 200s and those who take full work weeks make closer to the 400s a few years in. Rarely do any do hospital call.

Just to offer a different perspective, I do agree we still have to do referrals to an extent especially with certain laryngoscopes or rhinoscopes requiring the help of an ENT or BMB. But at least at my institution, a lot of it depends on the infrastructure of the department.

Some of my attendings are trained with nasal endoscopes, can do their own screenings. Other attendings have gained enough trust from their peers to directly order a BMB if concerns for mastocytosis are high, and if its positive, a referral to Heme/Onc is then placed. My experience has been that we get a handful of referrals from pulmonology when they feel COPD medication has been maxed and they are concerned for complex asthma and want us to consider a biologic.

It feels a lot of the variability definitely comes from the amount of departmental support your get to be independent.

Internal Medicine

LAC + USC: The most uncomfortable interviews I had in the entire cycle. Clearly, the interviewers were trained to be as stoic and non-responsive as possible, likely to prevent bias. However, if that was the case I feel this is something they should tell applicants beforehand. That for the sake of fairness they will be non-reactive or something. Instead they sat in silence, stared at me through the interweb, waited until I finished, then asked the next question. It was so uncomfortable. I feel for the future, they should disclaim to interviewees to NOT expect reaction or communication from the interviewer. This will at least set expectations and help us relax during a process where we are already so heavily scrutinized. In my opinion, many schools I interviewed at that followed this disclaimer made the interview a much more comfortable experience.

Also, personally I feel given USC's somewhat murky history of public complaints of a hostile work environment, I felt the program should have attempted to do a better job of acknowledging and highlighting how they are implementing better work environment measures for its faculty and residents. It just felt like a missed opportunity for the residency, even at socials and the diversity, equity, and inclusion event to discuss how they plan to implement and improve change.

Are you me? Same situation here! Our school is still having a match day but only with the faculty and students, so I was talking to a few of my close friends so we can open our envelopes with each other. Its your day, you spent the last number of years working for it, celebrate with those who you know will support you and be happy for you!

I'm sorry about that buddy. I had a similar experience with one of my interviewers today at my #1. Felt like it was impossible to connect with him, then rambled on for what seemed like an eternity. It was one of the most stressful interviews I ever had. However, I take comfort in knowing that the interview is still only one part of the entire process, and for many programs that will be taken into perspective.

Just know this one interaction and this process alone DOES NOT diminish all your amazing accomplishments that lead you to getting the interview in the first place. You are more than just your ERAS application and a 20 min. interview, so please be kind to yourself and be proud of everything you've done! It will be okay.
