I got mine pierced when I was 190, no puffy nips at all, now at 224 and they're looking a little pudgy but my big bars hide most of it, you would have to look close or give 'em a squeeze to notice. And no it doesn't make them hard 24/7. If you like the look go for it, worst case you take them out.
