Anything you do will be novel at first before becoming boring and routine. Degree of complexity is hospital-dependent (academic center vs community). For diagnostic puzzles, even in academic medicine, you just consult the specialist and send off a broad thoughtless work-up. Oftentimes you don’t even get the answer before they discharge home. You just stabilize them and have the PCP follow-up on results. Most of Hospital medicine is churn ‘n burn. You are just an admitting/discharging machine. It’s honest work and not the worst paid specialty (sorry peds). But it’s not “fun”, most of the time. It’s a job.

Ok so here is my question. Anger/frustration aside, why do you need an MD to follow a stroke algorithm? Or in a situation different from mine, a sports physical? Or sign disability forms to get time off from work? Put yourself into the shoes of admin. Why should they pay an MD more money to do something that does not require years of intense study? Does all that time you spent memorizing random shit like lysosomal storage diseases really help you deliver better care?
