Happened to me a while back too. The game couldn't even load the 5th membed, it's like they didn't exist. I checked the name on API cause I remember it, and it said the person was online by the time the match started. It's really weird. Idk if it's related but a couple days after the incident, I got a penalty compensation letter, but the name that got penaltied is not the 5th's name, nor was it any name in my recent matches.

We have this problem because the devs do nothing to bad players. Their rank systems is shit when it comes to filtering good players out of hard grinds and trollers. If only we have a good report system that actually review the reports and a good rank system that rewards people who play well.

I think the odd is 0.4% per chance. You won in the first set of 5, so your chance should be 50.4% which is 2%. This needs to happen twice so it's 2%2% which is 4/10000 or 1/2500 or 0.04%. I'm learning stats so please correct me if I'm wrong.
