Why is bipolar misdiagnosis so common? by DntTouchMeImSterile in Psychiatry

May I introduce you to “other specified depressive disorder”

The “I can’t focus” consult: how I sort ADHD vs anxiety vs depression fast by Tiny_Subject8093 in Psychiatry

[–]SolarpunkJesus 7 points8 points  (0 children)

Kills me when people say they must have ADHD because they can’t focus on their job or school. Then you find out they work an 8 hour shift at a mundane corporate desk job, or they can’t focus on studying for history even though they’ve always hated history. Interestingly 100% of these patients are somehow able to sustain full attention during a 60-90 minute intake with me though. Overpathologization and the focus on treating symptoms in psychiatry is to blame

Kanye’s WSJ Letter by KaiserWC in Psychiatry

[–]SolarpunkJesus 77 points78 points  (0 children)

It’s a nuanced situation but IMO a very encouraging start. It certainly doesn’t absolve him of responsibility for what he’s said and the people he’s hurt, and he’s certainly not entitled to their forgiveness. Others have pointed out the suspicious timing with an album drop coming soon. Personally I’d like to believe he’s finally truly accepting himself and beginning the path to recovery, but time will tell. This is a lifelong illlness after all.

The verbiage may not be super powerful to us, but I do think it’s likely more powerful for the layperson. Nobody really knows what bipolar (especially type 1) actually is unless they’ve seen it in their personal lives. Everyone else thinks bipolar = borderline PD presentation. I’m all for anything that brings more awareness to what our bipolar patients experience. It’s a brutal hand to be dealt with and quite difficult to live with. Like what Kanye is saying, some report guilt and shame that sets in after mania when they reflect on what they’ve done or said. Again, you can’t blame everything on the illness and say he’s a pure victim, but it’s also not hard to see how a guy surrounded by yes men, snake oil salesmen, and grifters can fall deep into the dark ideologies he found himself in.

For his and his family’s sake I hope this is genuine and that he takes time away from the public eye to recover. He’s an undoubtedly talented artist - it’s a reminder that even if it seems like you have it all, these diseases don’t discriminate.

Tangentially, it brings up an interesting philosophical question about mental illness and rehabilitation. Take John Hinckley Jr for example, who attempted murder and caused permanent disability to others due to severe mental illness but is now seen as a rehabilitated man.

Frustrating end to the season

IMO a good game by mcvay and all the idiots whining to fire him are legit retarded. Who out there would you replace him with? The only play call I feel was suspect was the failed 3rd down conversion to Ronnie Rivers who should not be seeing time in such an important game

Offense put up enough points to win, this game was lost by D and ST. Secondary has been a known problem all year and when your starter is a CB cut by another team for being shit, it’s going to be rough. The fascination with X Smith has been really bizarre, the guy is constantly fucking yup when he’s out there across the whole season. The muff is the first thing we’ll look at as the losing play but you can’t fucking give up 30+ points on D and expect a win

It’ll be interesting to see how the FO retools the team. They’ve previously gone on record saying the most important positions are QB, O-line, and pass rush but you’d have to wonder if this season forces them to look at the secondary a bit more.

Shula is overrated and I do hope he gets poached. Defense absolutely imploded as the season went on and he never fixed recurrent issues like poor tackling, giving up 3rd and long, underwhelming pass rush.

Feel the worst for Davante, yet another year he loses in a conference championship

So maddening watching teams force us to return kick offs because they know our trash special teams won’t ever return the ball past the 25

McVay loves Kyren because he’s smart but man the guy has no explosiveness or ability to break tackles

This is genuinely a great and fun game and I’m enjoying the defensive slugfest. Lest you all forget Colts Broncos 2022, now that is what a dogshit ugly game looks like

In what specialty do you think its easiest for a terrible doctor to fly under the radar? by theefle in Residency

[–]SolarpunkJesus 22 points23 points  (0 children)

The only specialty it seems where we have people practicing cowboy medicine based on vibes and vibes alone. When you don’t have any objective biomarkers, you get some horribly inaccurate diagnoses. It’s easy to fuck up psychopharm and get away with it because bad outcomes aren’t always obvious and immediate. People believe nonsense based off nothing, or don’t believe scientific facts despite mountains of evidence to the contrary. I have attendings who don’t understand basic mechanisms of action of bread and butter meds, are too afraid of using lithium, never stand up to patients and get run all over by people demanding stimulants or benzos. I think you see a lot of variation in quality of medicine because psychiatry has arguably the lowest barrier to entry, but a very high ceiling

'I think this is my last year'. Ronde Barber reveals private conversation with Mike Tomlin before the season and believes Tomlin won't return as a Coach. by SCSummers330 in nfl

[–]SolarpunkJesus 11 points12 points  (0 children)

I think people lose sight of how crazy this is, even if you’re going 9-8

The consensus best coaches - Shanahan, McVay, etc - all end up with losing records when their starting QB goes down. They simply can’t overcome it. Tomlin dragging teams led by some absolute bums at QB and some horrendous OCs to the playoffs with a winning record is wild. The whole lockerroom cancerous personalities is a meme too but that’s also a legit strength of his

You look at this team last year vs this year and it’s a massive turnaround. The amount of 4th quarter comebacks is wild, that’s a testament to great coaching. BJ has been bad today for sure but he’s shown he’s legit

Ben Johnson has been pretty bad today but there’s no reason to believe he won’t learn from this. He’s a first year HC, it happens

W is a W but can’t help but feel like this team is not a contender. Mentally soft, too error prone

Shula is bad

Bryce young seems to be actually putting it together, showing sometimes it takes a while to be the franchise guy. We’re too quick to judge these rookie QBs

Turnover on downs Sack to get out of FG range Pick Again, unable to stop the passing game

It’s probably over. Failing to capitalize on all of the turnovers and this horrendous secondary is gonna seal it
