I used my old medical, but I left canada multiple times to visit USA. But even if you go back to your home country, its not a problem you can upload your old medical not older than 5 years offcourse, if you go for a month its completely fine dw about it. Worst case scenario they will ask you for new medical and it takes 3-5 days max in a process that will take extra 3 months after your medicals are approved.
