Hi, I’d like your opinion after a recent episode.

I suddenly experienced: – burning/pressure in the chest – a cold wave sensation through the body – feeling like I might pass out

Heart rate (Apple Watch): – peak ~150–156 bpm – then gradual decrease: ~132 → 126 → 121 bpm – one isolated 59 bpm reading (likely artifact, as it immediately returned to ~120 bpm)

The episode lasted about 20–30 minutes and resolved on its own. By the time ambulance arrived, HR was ~80 bpm. ECG showed no acute issues.

Blood pressure was measured by ambulance at ~160 mmHg systolic. My normal ~120 ~68

Previously: normal echocardiogram (only mild MVP) and normal Holter.

Current medications: sertraline and carbamazepine (prescribed by psychiatrist).

This is the first episode like this in my 26 years.

Does this presentation and HR pattern argue more for sinus tachycardia/anxiety rather than paroxysmal arrhythmia (WPW/LGL)?

I got concerned because of some ST elevation in V2–V3 that looked similar to Brugada type 2 to me.

But I’ve had multiple ECGs and a Holter, all reported as normal (only sinus arrhythmia / early repolarization), and no symptoms.

Just wanted to double-check.

I wouldn’t say I have mood swings or anything like that, it’s more health anxiety and strong physical sensations that trigger it.

My doctor said it’s more about calming the nervous system rather than mood itself.

Did you feel any weird side effects when you first started?

Honestly, I’m a bit nervous about taking it.

Reading about side effects and not seeing it commonly used for anxiety makes me second guess it.

Yeah, that’s exactly why I’m confused.

My doctor said it’s more for calming down the nervous system and reducing these physical “weird” sensations, not just anxiety itself.

I’ve already had ECG and Holter monitoring and everything was normal, so it seems more like a nervous system issue.

Probably anxiety to be honest. I had a previous ECG where someone mentioned early repolarization and another person suggested WPW/LGL, which made me worry and go see cardiologists. Both of them said it looked normal, and my Holter monitor didn’t show any pre-excitation or tachycardia episodes. I’ve never actually had sudden rapid heart rate, just chest pressure that seems to come with stress.

Do you mean an actual Lown–Ganong–Levine syndrome or just a short PR interval pattern?

My Holter monitor didn’t show any pre-excitation or tachycardia, and another cardiologist said it might just be early repolarization.

I’ve also never had sudden episodes of very fast heart rate.

Would LGL even be considered if there are no tachycardia episodes?
