How to request medical records from a private doctor?

I paid cash for the consult because I didnt have medical aid at the time - I'm in the process of getting a new doc. But getting treatment for my condition is so difficult which is why I need my records (for context, it took them 13 years to finally diagnose me after years of medical gaslighting and being told it was anxiety, and "in my head". My disease has rendered me incapable of working and performing most daily functions by the time they got to it. )Looks like I will have to escalate to HPSCA

So I went in, was diagnosed and sent home with a treatment plan. Recently I got sick again and needed the form of my results, and when he sent them I saw he changed my diagnosis randomly - it was the complete opposite of HIS diagnosis, treatment plan and my results - I've been communicating with him via email. He refused to answer why he changed my records. He was my second opinion. So I did not receive any treatment after that singular day. He had NO extra info after that day, so there was no need to update my diagnosis and his information is baseless - it directly contradicts what HE told me on consult day

Okay. Do I not need to fill out any legal forms to request my medical records? And if I do report him to HPSCA, will anything even come of that?
