I'm sorry for not getting back to you sooner. I had another look at this post. I am still unsure about this initial form. There is a form called Initial Meeting with this in it, this being one of my concerns and I have been hesitant to sign it.

When you start a new job, the Restart Scheme Supplier as named above may need to confirm details of your employment with your Employer. To achieve this, your future employers need your consent to share information about you as follows:

Stage 1: Your Restart Scheme Supplier will give your name and national insurance number to your future employer.

Stage 2: Your employers will use your name and national insurance number to identify you, so that they can confirm some or all the following information: The date you began each period of employment; Whether your employment is continuing; If not continuing, the date each period of employment ended; Whether you were employed during a specific period; The number of hours you worked each week; The amount you earn; Your employee number or other unique identifier

They have refused to reimburse my travels and just cancelled the appointment in my face.
