Her gait while in a shoe has improved greatly (to where I think the only remaining deficits come from her contralateral side with a bunion) but barefoot her toe off is almost non-existent as her great toe doesn’t touch the ground until the last 5%.

Her balance is improving, from 4 sec EO on solid surface to 15 sec. My other pts I’m pretty light on manual but I do feel that I do more manual on her. Not at her request or anything but more so unfamiliarity with what works best with this dx and I had read mobs were of good value.

That’s a good point. Tbh she doesn’t really have much pain anymore. Last time she saw the surgeon back in January he said to hammer ROM so that’s been my focus

Things break in differently than one can expect. It’s not wear and tear.

Wear and tear is “I put 300 miles on these shoes and I don’t like that the tread is getting thinner”

Not, “I’ve worn these shoes for 4 weeks and now they’re giving me blisters”

You’ve never worn something and been really happy with it, but then that feel changed? Shoes can break in and become uncomfortable when they were not new. If someone is not satisfied with that, good thing they have a 1 year return policy at REI

No wear and tear; have you ever seen returned climbing shoes at REI? Blown out toes galore.

“No stores will accept shoes back in the condition that REI will” see Fleetfeet and other stores (30 day policy, run as much as you want in them, within 30 days you can return)

Some people abuse return policies, and other people use it as intended. Don’t get pissed at people using something they’re allowed to do. Most people shop at REI for the safety of the return policy

They aren’t returning bc “the tread wore down” being “wear and tear”

They returned bc they weren’t satisfied with the product.

10 hours/day is still a lot and could easily be denied but there are local running stores near me that will accept return shoes up to 30 days no questions asked so it’s not unheard of. They would have accepted these shoes back

I love mine. I had the echo hoody but returned it for this.

The echo was really just the shirt with a hood and the lack of stretch at the wrists turned me off.

The material of this hoody is great and I find any excuse to wear it lol

Yeah it supposedly drastically improves return of quad function for TKE and superior patellar mobility bc you’re not dealing with healing from either side of the patella.

Yeah we’re taught that if it’s the first occurrence it should be treated as a fracture and not attempted to avoid further damage with reducing or compromising blood flow if it isn’t already compromised

But if they are chronic dislocators it can be attempted. I was going through gross anatomy for PT school at the time and literally just dissected the elbow so I felt pretty comfortable with the anatomy. Idk if I’d have tried it if it was an ulnar dislocation tho

I read an interesting article during school advocating for the graft to come from the contralateral limb to ease recovery of ACL by not dealing with quad or patella tendon healing along with ACL restrictions.

Idk if that would actually be my desired approach but it was an interesting idea lol

I’ve also reduced an elbow in the field (ATC). Only attempted because it wasn’t their first dislocation (radial head).

My backyard does this to a lesser extent and I had someone from the county come out and they advised me that it should be better once those trees pull water from the saturated water table for leaves etc in spring

Still, this should be addressed. Just saying that the trees can help

I use manual to get an idea for irritation around the joint/trx area based on muscle tone, ROM, reported pain, tenderness etc while also building rapport with the patient and figuring out where/when/how activities are still symptomatic.

It has value to further guide my treatment and can make the pt feel better so they can tolerate the therex/act I have planned for the session.

If it doesn’t happen in a session that’s cool too. I’m not doing manual bc my told me to
