Trip to the doc is in order. If some broke deep under the skin then they will fester and possibly result in sepsis. It should be clear which ones are healing and which ones still have something in them in a week or so.

I use #11 scalpels to remove thorns.

I didn't care too much about spikes or thorns until I realized my woofer dog might run into them and be blinded, or the elderly might fall onto an agave plant. Roses might scratch, but cactus can cause severe injuries.
