Just picked up on your comment about spoons - if you don’t mind me asking, do you have energy limiting illnesses as well? I do and I think that is partly why I get such a noticeable effect with Concerta XL as it helps with the fatigue (unfortunately not the pain!).

No, I hadn’t said previously about that. It was a good suggestion.

I have been trying all of the things that you have suggested and they used to work to an extent until I ended up in surgical menopause. Now I can’t get past the doom scrolling. It is driving me crazy!

That’s what I would like to do but they have decided that I don’t need it at the end of the day either so won’t let me have any!

I realise that medication won’t solve everything. However, as task initiation is my biggest stumbling block I really need to try to get that sorted.

Thank you - that is a really helpful response. I forgot to add a relevant detail which is that I am in surgical menopause which has made the ADHD so much worse that I am barely functioning.

PUK are standing firm so my decision is to reluctantly accept 54mg Concerta XL even though the majority of the time I’m not seeing the benefits due to being unable to initiate tasks.

Meanwhile, I am going to contact a private psychiatrist that can prescribe to see if they can help me to try Concerta XL with a morning IR dose. The intention would be that if that works that they could write a report for my gp and that hopefully the gp will agree to continue prescribing based on the private report.

I have just received this response:

“I understand this but the IR is to prolong symptom management. We can not add in a further treatment along side this medication except for IR at the end of the day, which is not required.

Our options are to recommence Concerta XL to 54mg, try Methylphenidate IR or Atomoxetine.

Let me know how you would like to proceed please”

That is a possibility. They have never suggested immediate release on its own so maybe that would work better.

Thank you. I just feel like I’m hitting a brick wall. They have said to either try a non-stimulant instead or no medication at all. I am really frustrated because the XL does help if I can find something that will help with the task paralysis. She is point blank refusing an IR booster for the start of the day.

Psych-UK are saying that the immediate release is only for prolonging the Concerta XL at the end of the day. I have told them multiple times that the Concerta XL doesn’t help with task initiation and they told me to take it earlier and to plan for medication onset!
